Experience matters — especially when it comes to recognizing what is, and isn’t, endometriosis.

Some findings during surgery can look just like endometriosis implants — small, dark spots that resemble chocolate chips in a cookie. But not every one of these spots is disease. Some are simply blood clots from a prior surgery.

Mistaking these for endometriosis can lead to unnecessary excision — removing tissue that didn’t need to be removed. That’s why it takes real experience and a deep understanding of endometriosis in all its variations to know the difference.

When the disease is accurately identified and properly, safely removed, the prognosis is excellent. Getting the diagnosis right the first time protects healthy tissue and gives patients the best possible outcome.

What is a Cesarean Scar Defect (C-section “Niche”)?

Also known as a diverticulum, isthmocele, or even a uteroperitoneal fistula, a cesarean scar defect is a condition where the uterine incision from a prior C-section does not heal properly—leaving a pouch or defect in the uterine wall.

While often overlooked, this condition can have real consequences, including:
• Abnormal bleeding
• Pelvic pain
• Infertility
• Chronic inflammation

In some cases, it may even contribute to more complex conditions such as uteroperitoneal fistulas.

As discussed in my article in OBG Management, proper diagnosis and treatment are critical. Evaluation requires careful imaging and clinical expertise—and when indicated, treatment should focus on restoring normal anatomy through precise surgical repair.

Like many conditions in pelvic surgery, outcomes depend on experience, technique, and a comprehensive approach.

If you have cyclic pain or infertility and you’ve heard any of these phrases from your doctor, consider this your sign to trust your gut and get a second opinion. 🚩

Your pain is real, your symptoms are valid, and you deserve a medical team that truly listens. Here is the truth behind the myths:

“Your scans are normal” – Standard ultrasounds, CTs, and MRIs routinely miss endometriosis. A clean scan does not mean you are disease-free.

“Pain is normal” – Debilitating pelvic pain that disrupts your daily life is never “just part of being a woman.”

“Just take birth control” – Hormonal therapies only mask or suppress symptoms; they do not treat or eradicate the actual disease.

“I can cure your endometriosis” – Red flag! There is currently no definitive cure for endometriosis. While expert laparoscopic excision surgery is the gold standard for removing tissue and managing symptoms, any promise of a complete cure is misleading.

For decades, my life’s work has been dedicated to pushing the boundaries of minimally invasive surgery and championing the health of women suffering from endometriosis. Having performed these complex surgeries longer than almost anyone in the field, I have witnessed a truth that we can no longer afford to ignore.

Too many women are walking around with a devastating diagnosis: “Unexplained Infertility.”

Our research, published in reputable journals like Fertility and Sterility and Journal of Clinical Medicine, has proven time and again that a vast majority of patients with “unexplained” infertility or repeated IVF failures actually have underlying, undiagnosed endometriosis.

Unfortunately, there is still a significant gap in collaborative care. When endometriosis goes unrecognized, patients are often given aggressive fertility medications that can actually worsen their symptoms, only to be told later that they have no options left.

True patient care requires us to work together. I have even had specialists candidly tell me they hesitate to refer patients for surgery because once the endometriosis is treated, the patients conceive naturally. While a human reaction, our ultimate shared goal as physicians must always be the patient’s ultimate success and well-being.

Science is catching up, and the world is realizing what we have proven for the last 20+ years: proper, expert surgical treatment of endometriosis restores hope and builds families. Let’s bridge the gap between reproductive endocrinology and advanced surgery. Our patients deserve nothing less. 🩺✨

As I am often asked about it, I am happy to share my latest publication in the Journal of Clinical Medicine:
“The Revolution in Surgery That Saves Millions of Lives.”

This work reflects over 50 years of history—from the early days of video-assisted endoscopic surgery to the global adoption of modern minimally invasive surgery that has saved 10-20 millions of lives and has avoided major incisions surgeries in more than a billion patients.

What was once highly controversial when I introduced it to medical community is now the standard of care worldwide. Minimally invasive surgery has reduced complications, shortened recovery, improved quality of life, and helped save millions of lives—while sparing countless patients the trauma of large-incision surgery.

This journey has never been mine alone. I am deeply grateful to my patients, mentors, colleagues, friends, and especially my brothers.
