Position Summary
The LPN will provide skilled nursing care under the direction of an RN case manager, ensuring that each patient receives quality, individualized care in the comfort of their home health or hospice setting. This role includes collaboration with a multidisciplinary team to promote physical and emotional well-being for patients and their families.
Key Responsibilities
· Deliver skilled nursing care in accordance with physician orders and agency policies
· Monitor and report patient conditions and progress
· Administer medications and treatments as prescribed
· Provide education and emotional support to patients and families
· Communicate effectively with RNs, physicians, and the interdisciplinary care team
· Maintain accurate, timely documentation of all care provided
Qualifications
· Current Idaho LPN license in good standing
· Prior home health or hospice experience preferred, but not required
· Strong communication, critical thinking, and documentation skills
· Reliable transportation and valid driver’s license
· Compassionate and professional bedside manner
