Behind the scenes in the OR 🦴💪 A look inside a Reverse Total Shoulder Replacement with Dr. Scott Trenhaile.

So, what makes a reverse shoulder replacement different?

In a traditional shoulder replacement, the new components are designed to recreate the shoulder’s normal anatomy. With a reverse total shoulder replacement, the positioning of the ball and socket is switched. This changes the mechanics of the joint and allows the deltoid muscle to play a bigger role in lifting and moving the arm when the rotator cuff can no longer do its job effectively.

This procedure may be considered for patients dealing with:

• Severe shoulder arthritis combined with a large or irreparable rotator cuff tear
• Rotator cuff tear arthropathy
• Certain complex shoulder fractures
• Failed previous shoulder surgery or replacement
• Significant pain, weakness, and loss of shoulder function that hasn’t improved with conservative treatment

The goal? Reduce pain, restore function, and help patients get back to using their shoulder more comfortably in everyday life.

Ever wonder what actually happens during arthroscopic surgery? 👀

Come behind the scenes in the OR with Dr. Scott Trenhaile as we take a closer look at arthroscopy, a minimally invasive surgical technique that allows orthopedic surgeons to see inside a joint using a tiny camera called an arthroscope.

Instead of making a large incision, the surgeon works through a few small openings to evaluate the joint and, when needed, repair or treat the problem at the same time.

Arthroscopy is commonly used to treat conditions involving the knee and shoulder, including:

• Meniscus tears
• Cartilage damage
• Rotator cuff tears
• Labral tears
• Shoulder impingement
• Loose or damaged tissue within the joint

So, who might be a candidate?

Not every joint problem needs surgery. Arthroscopy may be considered when pain, catching, locking, weakness, or limited motion continues despite treatments like activity modification, physical therapy, medications, or injections, or when imaging and examination reveal an injury that may benefit from surgical treatment.

One of the biggest advantages? Because arthroscopy uses smaller incisions than traditional open surgery, many procedures can be performed on an outpatient basis with a recovery plan designed to get patients safely moving again.

Every injury, and every patient, is different. The first step is figuring out what’s causing the problem and which treatment actually makes sense for you.

A look inside the knee joint:

In this surgical footage, Dr. Scott Trenhaile performs a knee chondroplasty and partial meniscectomy while explaining what arthritis actually looks like inside the knee.

During a knee arthroscopy, chondroplasty can be used to smooth areas of damaged or unstable cartilage, while a partial meniscectomy removes the damaged portion of a torn meniscus while preserving as much healthy tissue as possible.

Dr. Trenhaile also discusses an important part of the picture: knee arthritis and cartilage damage. Meniscus tears, cartilage wear, and arthritis can occur together, which is why understanding the entire joint, not just one MRI finding, is important when determining the right treatment approach.

Great day at the OLC in Chicago! Always a privilege to connect with fellow orthopedic surgeons, share techniques, and continue advancing the way we treat rotator cuff pathology.

Today’s training focused on the TENDON SEAM technique, with great discussion, hands-on learning, and time in the cadaver lab. I always enjoy the interaction and exchange of ideas that come with these courses.

Continuing to learn, teach, and refine our techniques ultimately means better care and better outcomes for our patients.
