Can saline breast implants be removed en bloc when the capsule is extremely thin?

In this video, Dr. Philip Straka demonstrates a recent en bloc breast implant removal involving intact saline implants and very thin surrounding capsules.

Although the scar tissue was nearly transparent, Dr. Straka was able to remove each implant and its capsule together in one piece. After opening the specimens, both saline implants were found to be clear and intact.

This case shows that capsule thickness alone does not always determine whether en bloc removal can be completed. Every procedure must be approached individually based on the patient’s anatomy, implant type, surgical history, and what can be performed safely during surgery.

Dr. Straka focuses exclusively on breast implant removal and helps patients understand their explant options, including whether en bloc capsulectomy may be appropriate for their specific case.

What does an intact breast implant look like after en bloc removal?

This image shows the breast implants after they were removed during an en bloc breast implant removal procedure. In this case, both silicone implants were found to be intact, even though the surrounding capsule was extremely thin.

One of the most common questions patients ask is whether it’s possible to know if an implant has ruptured before surgery. While imaging can provide valuable information, the implant’s condition is ultimately confirmed once it has been removed and examined.

Every breast implant removal procedure is unique. Factors such as the condition of the implant, the thickness of the capsule, and a patient’s surgical history all help determine the most appropriate surgical approach.

Dr. Straka focuses exclusively on breast implant removal and has extensive experience treating patients seeking explant surgery for a variety of reasons. During your consultation, he will discuss your goals, review your imaging and history, and recommend the approach that best fits your individual situation.

Not ruptured… but not normal either.

These implants were removed en bloc, meaning the implant and capsule were taken out completely intact. On the surface, everything may appear “fine”—but a closer look tells a different story.

Both implants showed visible creasing, which is where the shell weakens over time. This is often how leaks begin—not always with a dramatic rupture, but with subtle structural changes that you can’t feel or see from the outside.

Even more interesting—
The capsule surrounding the implants was extremely thin, almost transparent, yet still strong enough to be removed intact.

What this shows 👇
“Intact” doesn’t always mean ideal.

And aging implants can change long before symptoms appear.

Every patient’s decision is personal—but understanding what’s happening inside your body matters.

If you’ve had implants for years and are starting to question them, it may be time to explore your options.

Breast implant removal—also known as explant surgery—is a deeply personal decision, and one we’re seeing more women explore.

There isn’t just one reason.

For some, it’s a shift in lifestyle.

For others, it may be related to implant aging, rupture, or changes in how they want to look and feel.

And for some, it’s about addressing symptoms they believe may be linked to their implants.

What matters most is this:
Your decision should be informed, supported, and completely your own.

At our practice, we take the time to understand your goals, answer your questions, and guide you through every option—whether that includes removal, replacement, or revision.

If you’ve been thinking about explant surgery or simply want to learn more, we’re here to help.

These implants were still intact—but that doesn’t mean everything was okay.

In this case, one implant showed a deep crease, indicating it was likely on the verge of rupture, while both were affected by significant capsular contracture.

When scar tissue tightens around the implant, it can distort shape, increase pressure, and weaken the integrity of the implant over time.

👉 This is how problems can develop before an actual rupture occurs.

As shown here, both implants were removed intact during en bloc removal—ensuring the surrounding capsule and any affected tissue were taken out together.

The takeaway:
Just because an implant hasn’t ruptured doesn’t mean it’s healthy.

If you’re noticing changes in shape, firmness, or discomfort, it’s worth getting evaluated.
