What happens when a silicone breast implant ruptures—and why might en bloc removal be used?

In this video, Dr. Philip Straka examines implants immediately following an en bloc breast implant removal procedure. What he finds demonstrates just how different the condition of two implants can be in the same patient.

On one side, the silicone implant was completely ruptured inside the surrounding capsule. On the other, the implant remained intact, although Dr. Straka observed scar tissue adhering tightly to the textured silicone implant.

In this particular case, removing the ruptured implant en bloc meant the implant and intact surrounding capsule were removed together before the specimen was opened, helping contain the ruptured silicone during removal.

Silicone implant rupture isn’t always obvious before surgery. The FDA notes that some silicone implant ruptures are “silent,” meaning a patient may not notice symptoms or changes when a rupture occurs.

What happens to your breasts after breast implant removal?

It’s one of the biggest questions patients have before explant surgery—and the answer is different for everyone.

After breast implants are removed, the breasts lose the volume the implants previously provided. Depending on your implant size, skin elasticity, natural breast tissue, age, weight changes, pregnancy history, and how long you’ve had implants, you may notice changes in breast shape, fullness, or skin laxity. The FDA notes that some patients may experience sagging, dimpling, puckering, or other cosmetic changes after implant removal without replacement.

Another common question is: Do I need a breast lift after breast implant removal?

Not necessarily. Some patients are comfortable with their natural breast shape after explant surgery, while others may benefit from a breast lift to address excess skin or reposition the breast. Dr. Straka can also perform a breast lift at the same time as explant surgery when appropriate.

What’s the difference between en bloc breast implant removal and a complete capsulectomy?

Although the terms are sometimes used interchangeably, they describe two different approaches to breast implant removal.

In this video, Dr. Philip Straka shows the difference using specimens from an actual explant procedure:

🔹 Complete capsulectomy: The entire capsule, or scar tissue surrounding the breast implant, is removed. However, because there is an opening in the capsule, it is not considered an en bloc removal.

🔹 En bloc removal: The breast implant and the entire surrounding capsule are removed together, with the capsule remaining intact around the implant.

In the case shown here, 100% of the capsule was removed on both sides. The difference is whether that capsule remained completely intact during removal.

Understanding terminology like en bloc capsulectomy, complete capsulectomy, and explant surgery can help patients have more informed conversations about their breast implant removal options.

Why do my breast implants feel like they’re burning?

A burning sensation around breast implants can have several possible causes, including capsular contracture, implant rupture, seroma, or nerve-related sensations during healing. Some patients who report symptoms associated with Breast Implant Illness (BII) also describe burning or tingling around their implants.

While some sensations can occur during normal recovery, new, persistent, or worsening burning should not be ignored—especially when accompanied by swelling, redness, firmness, changes in breast shape, fever, or increasing pain.
