Vascular surgeons didn’t spend 10+ years training to surrender clinical autonomy to corporatization. Yet as healthcare becomes increasingly consolidated, vascular surgeons have fewer choices in how they practice and less control over the decisions that shape their work.

More than 75 million Americans currently live in a county without a vascular surgeon. And vascular surgery is projected to have the largest physician shortage, meeting just 66% of demand by 2038.

We need to grow the vascular surgery workforce. We also need to make sure the vascular surgeons already in practice can stay there.

That means keeping vascular surgeons at the center of the clinical and operational decisions that shape patient care.

It also means preserving choices in practice setting.

Private practice must remain a viable option. But every day, it is becoming harder to sustain.

Rising costs, declining reimbursement and administrative burden are putting greater pressure on independent practices, while continued consolidation leaves physicians with fewer options.

SVS advocates every day on the payment, regulatory and practice issues affecting vascular surgeons and provides resources to support members in every practice setting.

We know that does not make the current realities of independent practice any easier.

If we want patients to have access to vascular care, we have to make sure vascular surgeons can stay in the workforce, retain clinical autonomy and have viable choices in how they build their careers. That is what SVS will continue working to protect and support.

Yale New Haven Hospital's Brandon A. Creisher, MD, presented "Outcomes of infrainguinal bypass with spliced veins in patients with chronic limb threatening ischemia" at .

Objectives ⬇️
Single segment greater saphenous vein (ssGSV) is the gold standard conduit for infrainguinal bypass in chronic limb-threatening ischemia (CLTI). Spliced vein bypass (SVB) is an alternative conduit, but its outcomes have not been well characterized. Moreover, the impact of the number of spliced segments on outcomes remains unclear. This study examines the outcomes of SVB with 2 vein segments (2sSVB) and 3 vein segments or more (3sSVB) compared to ssGSV for infrainguinal bypass.

Methods ⬇️
The Infrainguinal Bypass Vascular Quality Initiative (VQI) module (2003-2025) was reviewed for patients treated for CLTI with ssGSV, 2sSVB, and 3sSVB. Propensity matching (3:1) was performed to compare 2sSVB (cohort 1) and 3sSVB separately to ssGSV (cohort 2) adjusting for baseline characteristics including indication, urgency, and bypass configuration. Perioperative and long-term outcomes were reported, and the primary outcome was MALE-free survival.
