We are currently hiring an ECM Case Manager serving clients throughout Redding and Eastern Shasta County.

✨ What You’ll Do:
• Support clients with healthcare, housing, benefits, and community resources
• Collaborate with medical, behavioral health, and community partners
• Build meaningful relationships and provide client-centered care
• Conduct outreach and care coordination in community settings
• Help improve health outcomes and quality of life for rural communities
✨ What We’re Looking For:
• Experience in case management, healthcare, behavioral health, or human services
• Strong communication and organizational skills
• Comfortable with documentation and electronic health records
• Compassion for serving diverse and underserved populations
• Ability to work independently and as part of a collaborative care team
