Your ACL: What to Know Before Surgery
If you just tore your ACL, you are probably somewhere between shocked and overwhelmed, and you likely have a surgery date already or one coming soon. We want to walk through a few things that will help you feel more prepared, because there is a lot you can do in this window.
Start with this. Not every ACL tear requires surgery.
For younger athletes returning to cutting and pivoting sports, reconstruction is usually the right call. For people whose activities are more straight line, some do very well with rehab alone. That conversation belongs between you and your surgeon, and it is worth having rather than assuming.
If surgery is the plan, the weeks before it are extremely valuable. Quadriceps strength going into the operation is one of the better predictors of how the knee functions two years down the road. That is one of the most consistent findings we have in ACL research.
Getting the swelling down, getting the knee straightening fully, and waking those quads back up before surgery genuinely pays off later.
There is one thing we want you to hear clearly. Getting your knee to straighten all the way, both before and after surgery, matters enormously. It is the piece people underestimate most, and it is the hardest one to recover if it gets away from you early.
Now for the timeline. Return to sport is not a date on a calendar. It is a set of milestones your knee has to meet, including strength testing, hop testing, movement quality, and confidence.
Athletes who return based on meeting criteria rather than on hitting a certain month have meaningfully lower reinjury rates. Rushing back costs more than waiting does!
That last part is where the mental side comes in, and it is real. Trusting your knee again takes time, and fear of reinjury is one of the strongest predictors of whether someone actually returns to their sport. We work on that alongside the strength, because both have to come back together.
Here is what we want you to take from this. You have more influence over this outcome than you might think. The work starts now, not after surgery, and you do not have to figure it out on your own.
