For 50 years, Northern Counties Health Care has been honored to provide compassionate, high-quality care to the people of the Northeast Kingdom. What began as a vision to make health care more accessible has grown into a network of providers, clinicians, and staff committed to improving the health and well-being of our communities every day.

As we celebrate this milestone, we're grateful to our patients, employees, partners, volunteers, and supporters who have been part of our journey. Thank you for helping us build healthier communities over the past 50 years—and for being part of what comes next.

The Northern Counties Board of Directors regularly recognizes those who show outstanding dedication to our organization and communities. We’re proud to recognize Breanna Barnes, whose compassion, kindness, and dedication make a meaningful difference for the patients and families she serves through Caledonia Home Health Care & Hospice.

As a hospice aide, Breanna provides more than exceptional hands-on care—she offers comfort, dignity, and reassurance during some of life’s most challenging moments. Her calm presence, reliability, and genuine compassion recently inspired a heartfelt letter from a patient's spouse, who shared how Breanna's weekly visits not only brought comfort to her husband but also provided her with a much-needed opportunity to step away from her role as a full-time caregiver, knowing her loved one was in caring hands.

Breanna's commitment reflects the extraordinary work of our Licensed Nursing Assistants and Personal Care Attendants, whose impact is often felt far beyond the care they provide. Thank you, Breanna, for your compassion, dedication, and the difference you make in the lives of our patients and their families every day.
