PACE of Southwest Michigan The Program of All-Inclusive Care for the Elderly (PACE) helps older adults remain at home by providing integrated medical and social services.

PACE (Program of All-inclusive Care for the Elderly) is a unique program of long term care that allows elderly adults to remain in their home. We work with you and your loved ones to provide the support at home, medical treatment, and social interaction needed to remain strong and independent. The key to your success is our care team. This is a team of physicians, nurses, pharmacists, social worke

rs, therapists, and transportation staff who are familiar with you and your needs. The care team meets daily to discuss all aspects of your care and provide services when appropriate. Our physician is a geriatrician specializing in the care of adults as they age. We encourage caregivers to remain involved by providing frequent communication and supportive services including respite care. PACE of Southwest Michigan serves Berrien, and parts of Cass and Van Buren Counties. Note: A PACE participant may be fully liable for the cost of medical services from an out-of-network provider or without previous authorization (except for emergency services).

Our PACE team and participants enjoyed a wonderful field trip to Fernwood Botanical Garden in Niles, MI! ๐ŸŒธ๐Ÿฆ‹

We had a delicious lunch, the weather was absolutely perfect, and everyone enjoyed exploring the beautiful gardens filled with colorful flowers and butterflies. Most importantly, our participants had an amazing time making memories together.

It was truly a wonderful day filled with laughter, friendship, and the beauty of nature! ๐Ÿ’š

We've been busy, to say the least, but what we experienced last Thursday was truly fulfilling. It was uplifting, comforting, and a reminder of why we do what we do. It filled our hearts and our cups.

Last Thursday, alongside our community members, Board members, Chamber members, the wonderful Niles community, and our dedicated team, we celebrated the Ribbon Cutting Ceremony for our new Niles location.

This milestone didn't happen overnight. It came after years of hard work, challenges, and perseverance. Through every trial and tribulation, our team remained resilientโ€”and together, we made it happen.

It is an incredible honor to expand our services in the Niles area and have the opportunity to care for up to 150 seniors in Berrien and Cass counties, helping them remain safely and independently in the homes they love.

Thank you to everyone who attended our celebration and to everyone who played a part in making this milestone possible. Your support means the world to us, and we are excited to begin this next chapter of serving our community.

Stony was selected for this special honor because his nomination reflected his unwavering dedication to both his family and the greater Tri-City area.

Stony is a LIFECARE driver who provides transportation services for PACE of Southwest Michigan. We are incredibly proud to have him as part of our PACE team. We were honored to join the PACE Association of Michigan and more than a dozen other PACE organizations for another successful PACE Day at the Capitol!

Several of our participants and team members from PACE of Southwest Michigan had the opportunity to meet with legislators and share the value and impact of the PACE model of care. Together, we advocated for older adults across our state who want to remain healthy, independent, and in the comfort of their own homes with the support of PACE services.

Last week, members from the PACE Association of Michigan visited Lansing to wrap and deliver 200 PACE Day at the Capitol cookies to legislators!

The โ€œSave the Dateโ€ cookies were dropped off to remind representatives to join seniors and team members on the Capitol lawn for an eventful PACE Day at the Capitol on Wednesday, June 10th.
