Recently took on a tough referral from Ortho: a diabetic patient with neuropathy and multiple comorbidities who had a failed ORIF of a trimalleolar ankle fracture with subsequent removal of fixation and placement of external fixation before she was sent to me.
Instead of a traditional open fusion, I used a minimally invasive technique to prepare the ankle and subtalar joints followed by an intramedullary nail. This approach minimizes soft tissue trauma — critical in high-risk patients with poor healing potential.
Grateful to report she’s doing very well so far with stable fixation and has started protected weight bearing.
