Dr. Bindewald is a double board certified plastic surgeon based in San Antonio, TX. He is the head of the San Antonio Plastic Surgery Center, which has been well-known in San Antonio for decades. He completed his undergraduate training at the University of Colorado, where he graduated with a Bache

lor of Science in chemical and biomedical engineering. Dr. Bindewald then returned to Hawaii for medical school, graduating from The University of Hawaii John A. He completed general surgery training in Santa Barbara, CA. This is also where he met his wife, also a well-respected surgeon in San Antonio. He served as a general surgeon in the United States Navy for four years before returning for additional training in Plastic and Reconstructive Surgery at the University of Texas Health Science Center.

Abdominoplasty is not one-size-fits-all.

There are many variations of a tummy tuck, and the right approach depends on each patient’s anatomy, goals, and degree of skin and muscle laxity.

For some patients, the procedure may involve removing overhanging skin. For others, it may require a more extended or circumferential approach. Many abdominoplasties also include liposuction to improve contour, and the amount of abdominal wall tightening can vary significantly from person to person.

This is why a customized surgical plan is so important.

In this case, our patient underwent removal of nearly 7 pounds of excess tissue, liposuction of an additional 600 cc, and repair of a 5 cm diastasis of the abdominal muscles. She is just 9 weeks post-op and already thrilled with her transformation.

Benefits of Breast Reduction

Breast Reduction and Lift 

Although many people think of breast reduction as simply making the breasts smaller, the procedure typically includes a significant lift. Body contouring is enhancing one’s form.

We see patients interested in these services after weight-loss, whether it is due to medications, surgery or through diet and exercise.

Weight-loss affects many areas of the body. Some of the more common changes seen after weight loss include:
- Persistent areas of fatty tissue
- Excess skin around the belly trunk
- Skin laxity along the back or sides of the chest
- Skin laxity along the arms and thighs
- Deflated appearance of the chest (both men and women)

There are many techniques that can be used to address these areas:
- Liposuction alone or in combination with other procedures
- Panniculectomy, abdominoplasty, extended abdominoplasty, fleur-de-lis abdominoplasty and lower body lift
- Bra-line lift or torsoplasty
- Thigh lift or arm lift (brachioplasty)
- Breast lift (mastopexy) with or without augmentation

The use of these techniques is custom tailored to each of our patients and their goals and priorities.

Our wonderful patient first came to see us before losing weight. We discussed her objectives and options. She returned after losing weight via bariatric surgery. She was primarily concerned with the loose skin on her abdomen and hips as well as the deflated appearance of her chest. She underwent an extended abdominoplasty with augmentation mastopexy. Reasons for Augmentation Revision:

1) Capsular Contracture:
 Capsule formation: A capsule is the normal layer of scar tissue that forms around a breast implant.
 Capsular contracture: When this scar tissue thickens, it is called capsular contracture.
 Severe cases: In more advanced cases, the breast may become painful or distorted.

2) Implant Rupture:
 Rupture risk: Saline and silicone implants can rupture over time.
 Signs and detection: A saline implant rupture is often noticeable right away, while a silicone implant rupture may appear as worsening capsular contracture or require ultrasound or MRI for diagnosis.

3) Implant Position:
 Position changes: Implants may shift downward or to the side over time.
 Revision options: Surgery may be needed to reposition the implant and reinforce the pocket, sometimes with mesh support.

4) Lift:
 Skin changes: Breast skin may stretch or loosen over time, especially after pregnancy or weight changes.
 Purpose of a lift: A breast lift removes excess skin to create a firmer, more youthful appearance.

5) Size:
 Size concerns: Implants may be too large, too small, or uneven in size. Preoperative planning: Surgeons work to reduce this risk by carefully reviewing the patient’s goals and selecting an appropriate implant size before surgery.
 Changes over time: Preferences or breast appearance may change later, which can lead to revision.

Our lovely patient developed a severe capsular contracture 10 years after breast augmentation performed elsewhere, causing pain and breast distortion. Implant exchange with capsulectomy relieved her discomfort and restored a softer, more natural breast shape. She chose not to have a lift to avoid additional scars and is very pleased with her revision results.

The benefits of abdominoplasty (tummy tuck):

1) Removal of loose skin
2) Repair of the abdominal muscles
3) Elimination of stubborn fat
4) Possible improvement back pain

How this is accomplished:

1) Excess skin is removed through a low incision designed to be hidden under underwear or a bikini bottom. To keep the scar low, a short vertical scar may sometimes be needed to close the former belly button site, especially in patients with a high belly button or limited loose skin, as shown in this case. Placing the incision too high or removing too much skin can result in a higher, more noticeable scar.

2) The muscle fascia is brought together with an internal corset-like repair. This realigns the abdominal muscles and helps to develop a slenderer waist.

3) These are treated via targeted liposuction during the procedure. Examples of this can be seen in this case in the preoperative markings.

4) This may improve as the core muscles are tightened. However, removing too much skin can create a forward-hunched posture, which may worsen low back pain. Achieving the right balance is essential!

Our goals in any abdominoplasty are to use these three techniques (skin removal, muscle repair and liposuction) to safely improve the appearance and function of the abdomen!

Sometimes things can be challenging.

This young lady had underdevelopment of the left side of her chest (Poland's syndrome) with an absent pectoralis muscle. She also had a relatively ptotic breast on the right with an enlarged ar**la. She was looking for enlargement and improved symmetry.

We gave her a dual-plane augmentation on the right with a 240cc implant and a lift (augmentation/mastopexy) and a subglandular augmentation on the left with a 375cc implant. We also reduced her ar**la on the right.

At just 3 months, we achieved her goals. I expect her symmetry to improve as the implant on the left settles and her scars fade.

Gynecomastia correction is one of the most frequent procedures we perform in our gentlemen patients.

All men have breast tissue! Gynecomastia is the overexpression thereof. This can lead to self-consciousness regarding clothing, activity and intimacy.

There are three main factors that determine how this can be corrected:

1) The extent of the breast tissue
2) The consistency of the breast tissue
3) The extent of extra skin
