"I feel normal for the first time in my life."

She'd been embarrassed about her body since she was old enough to notice she was different from other women. That's not months. That's most of a lifetime.

What she wrote about afterward wasn't dramatic. Minimal discomfort. Feeling normal.

She's a nurse, and she made a point of mentioning that Dr. Hailparn called her the night of her surgery and gave her a cell number. Her words: very few physicians do that.

There's a difference between not knowing help exists and being convinced it doesn't. The second one is harder, and it's more common than it looks — women reason their way to it after years of assuming this is just their body and nothing can be done about it.

S*x doesn't feel the way it used to.

Arousal takes longer, or doesn't happen. Or***ms are weaker or out of reach. Interest has faded. Or in*******se has become uncomfortable in a way it never was.

Somewhere between 40 and 50% of women experience this at some point. Almost none of them get told why.

The causes are usually identifiable. Declining estrogen thins tissue and reduces blood flow and nerve sensitivity. Vaginal delivery can affect nerves, leave scar tissue, or change tone. SSRIs delay or block or**sm in 30–70% of women taking them; a side effect many are never warned about. Blood pressure medications, hormonal contraceptives, and antihistamines all contribute. So do diabetes, thyroid disorders, and chronic pain.

And some of it isn't physical at all. Stress, depression, relationship strain, and past trauma affect sexual response in ways no procedure can reach.

That distinction matters more than any treatment decision. The right answer depends entirely on the cause — and it's the question that almost never gets asked.

If you take allergy medication regularly, this is worth knowing.

Antihistamines and decongestants work by drying out mucous membranes. That's the mechanism — it's what makes them effective on a runny nose.

Your body doesn't apply that selectively. The same drying happens everywhere you have mucous membranes.

Dr. Hailparn: women taking allergy medication on a regular basis often don't connect it to vaginal dryness. Caffeine compounds it, since it's dehydrating.

None of which means stop your allergy medication. It means that if you're dealing with dryness and can't work out why, the answer might already be in your medicine cabinet — and that's a conversation worth having with your doctor.

That's a patient's advice, and it's good advice.

Volume matters in this procedure. So does whether the surgeon will tell you what recovery genuinely involves, and what the plan is if something needs revision.

Ask. A surgeon who takes those questions seriously is showing you how she'll handle the rest of it.

The Global Meno Educa-thon runs October 10–17 — seven days of menopause and women's health education, with 20+ speakers from the US, UK, Canada, India, Kenya, Nigeria, Cameroon, Ghana, South Africa, and Tanzania.

Dr. Hailparn is speaking on lichen sclerosus, which fits: October is also LS Awareness Month.

Her words: if you've been living with this kind of thing, whatever it is, you don't have to wait as long as she did.

That's the part worth repeating. Not the surgery. The years before it.

Long, bulky l***a causing pain, discomfort, and embarrassment is not a rare problem. Staying quiet about it for decades isn't rare either.

If something has bothered you since you were young and you've never raised it with anyone, that's worth a conversation.

That's the most common thing women say afterward. It's also the one question that's hardest to get an honest answer to beforehand: what does ThermiVA actually feel like?

Here's the walk-through.

No preparation is needed — eat normally, take your usual medications, come in on a lunch break if that's what works. The treatment room is a standard exam room. You're positioned as you would be for a routine gyn exam, and no speculum is involved.

A slim handpiece delivers radiofrequency energy to the vaginal tissue. What patients feel is warmth — a gentle, spreading warmth that most describe as a warm massage. That portion takes 15–20 minutes. The external tissue takes another 10–15.

The device monitors tissue temperature continuously, holding it in the therapeutic range that stimulates collagen production without reaching the point that causes pain. No cutting, no needles, no ablation.

Total time: 30–45 minutes. No recovery period, no restrictions, no downtime.
