Bone density does not always tell the full story. In diabetes, bone quality can be affected even when bone scans appear normal. That means bones may be more fragile than bone density alone would suggest.

Chronic high blood glucose can lead to the buildup of advanced glycation end products (AGEs), which alter collagen cross-linking inside bone. Over time, this can reduce bone flexibility and strength. Bone turnover may also slow down, leaving older, more brittle bone in place longer. In addition, microvascular changes may affect how bone tissue remodels and repairs itself.

There is also a higher risk of falls due to neuropathy, vision changes, or hypoglycemia, which further increases fracture risk.

In type 1 diabetes, bone density is often lower and fracture risk is significantly higher. In type 2 diabetes, bone density may appear normal or even elevated on scans, yet fracture risk still increases.

The encouraging part is that you can take steps today. Stay consistent with blood glucose management, include strength and weight bearing exercise, support your body with adequate protein, calcium, and vitamin D, and talk with your healthcare team about bone health screening. Constant blood sugar checks, insulin dosing, carb counting, and unpredictable highs and lows can feel exhausting, especially when hormones during puberty make everything even harder. Many teens experience anxiety, frustration, or full burnout, leading to “I just can’t deal with this anymore” moments.

They’re not lazy or failing. Managing diabetes while navigating school, friends, and growing up is a lot. Here are some tips on what to say when they are feeling overwhelmed:

• Talk openly with your diabetes team about how you’re really feeling and consider speaking with a counselor who understands diabetes distress
• Connect with other T1D teens
• Use technology (CGMs, automated pumps) to lighten the daily load
• Simplify where you can. Repeat meals, lean on technology like CGMs.
• Focus on the basics: taking insulin and checking blood glucose at key times. That alone is a strong foundation

Small steps and support can help regain motivation and mental health matters just as much as their A1C.
