Surface ablation surgery (PRK) is frequently preferred for an enhancement or touch-up procedure because it avoids cutting a new corneal flap. PRK is a surgery often done when LASIK surgery is not viable to the cornea. PRK can be done for people who have less tissue in the cornea, not enough to create a flap in the eye that is often attributed to LASIK. PRK is more common than not in older patients who have had their tissue degrade over time. Although different from LASIK, it is just as effective and essentially create the same result.

Keratoconus is often treated with corneal cross-linking where riboflavin is used and activated under UV light to stop the progression but does not correct vision. This video shows the way in which trans-epithelial topography guided laser followed by cross-linking, the CREATE+CXL Protocol, is used to potentially restore one's vision close to normal.

This patient of ours were just recently diagnosed at the young age of 23 and immediately seeking out treatment to help fix his vision after a month of scleral lenses. Now, those sclerals are no longer needed!

We have a new publication! In it we demonstrate how the epithelium thins over a keratoconus cone, and thickens around the periphery of a keratoconus cone. This could be used as part of the diagnosis of keratoconus, as that pattern is very indicative of corneal ectasia.

It also definitively affects surgical planning for a procedure as that area of epithelial thinning hides that irregularity from topography measurement causing incorrect mapping of actual stroma HOA.

We used this data for our CREATE Protocol, which essentially combines an epithelial compensation adjusted trans-epithelial PTK with topography guided PRK. Without such a procedure, keratoconus treatment is less effective and can leave behind a “mini cone.”

A LASIK enhancement (or a laser touch-up using LASIK or PRK) can safely correct residual irregularities in the eye if your cornea is stable and sufficiently thick. The procedure takes seconds, relifting the lasik flap and reshaping the cornea to fix remaining irregularities. The final result is to get a patient at or close to 20/20 vision as possible.

The CREATE Protocol performed by Dr. Motwani is for repairing this patients prior Radial Keratotomy (RK) and is typically a specialized two-step laser procedure designed to address the severe corneal irregularities and vision deterioration that often occur years after an RK surgery.

While Part 1 focuses on reshaping the cornea to make it uniform and removing higher-order aberrations, Part 2 specifically targets the refractive power.

Objective: To correct the overall refractive power of the cornea and eliminate remaining refractive errors (such as severe farsightedness, nearsightedness, or astigmatism) left behind from prior RKs, corneal scarring, eye trauma or disease/infection after the corneal shape has been normalized.

Timing: Part 2 is performed months after Part 1. This intentional delay ensures that the corneal tissue and incisions have completely healed and stabilized from the initial reshaping before the final vision correction power is applied.

The Goal: By separating the structural smoothing from the power correction, the protocol aims to safely restore stable visual clarity and bring the patient as close as possible to 20/20 vision without the need for rigid corrective lenses.

The IC-8 Apthera IOL is an FDA-approved, small-aperture extended depth of focus (EDOF) intraocular lens used during cataract surgery. It features an embedded mini-ring that creates a pinhole effect, filtering out peripheral unfocused light to provide a smooth, continuous range of sharp vision from near to far.

Intraocular lenses, while they are an option for those to help see better, becomes a sort of avenue for discomfort as time goes on and the pinhole itself, while efficient in high exposure environments can be a problem in dimmer and darker places when the pinhole itself is to small to allow light in the eye.

For patients suffering from ectatic disorders like keratoconus, the natural cornea becomes dangerously thin and bulges outward. Inserting this extra biologic tissue, like CTAK, does not add viable corneal tissue as many may believe. The tissue in CTAK's have been gamma radiated away and what is left is the physically integrity of the tissue- leaving behind no living tissue.

CTAK also poses for issues later on when one chooses to do scans on the eye and get corneal reconstruction because the irregularity of the cornea caused by the CTAK.
