Throwback Thursday: Special Needling for the Knee

Every Thursday, I share a page from Dr. Richard Tan’s original case notes—a tradition inspired by my dear friend Mark Montoya, whose dedication helped preserve and spread the Balance Method throughout our community. ❤️

This week’s case takes us back to 2007. In the first note, Dr. Tan documented pain behind the left knee, accompanied by sciatica. Here, he applied imaging: the left knee was reflected onto the right hand, with the second joint of the index finger corresponding to the knee.

Also notice the way Dr. Tan used Shou Jing Wu to treat pain on the dorsal side of the foot. Even when a patient reported pain in both knees, Dr. Tan rarely needled both sides.

I feel incredibly grateful to have inherited so many of Dr. Tan's personal case notes and teaching materials. Every page continues to deepen my understanding of his clinical thinking and influences how I practice, teach, and carry forward the Balance Method today. 🙏

Every Thursday, I share a page from Dr. Richard Tan’s original case notes—a tradition inspired by my dear friend Mark Montoya, whose dedication helped preserve and spread the Balance Method throughout our community. ❤️

This week’s case is from January 2014, involving a patient experiencing pain around the knee, upper leg, and hip. It’s also a wonderful example of something Dr. Tan always taught us: Make the treatment simple.

When a patient comes in with pain, we follow Acupuncture 1-2-3:
1️⃣ Identify the sick meridian. Where exactly is the pain located?

2️⃣ Balance the meridian. Use the Five Systems to determine the balancing meridian.

3️⃣ Select the points. Use the Imaging and Mirror concepts to find the appropriate points.

You can see this process in Dr. Tan’s notes. For the Yang Ming hip and leg pain, he used direct imaging on the upper arm. For the hip joint itself, he used reverse imaging at ST 41.

It was reported that after one treatment, the patient was feeling better, particularly along the lateral side. In the follow-up visit the patient reported extra Foot Shao Yang GB hip pain. Dr. Tan added the San Jiao Meridian to balance it.

Simple and precise. That was Dr. Tan. He truly practiced what he preached.

Every Thursday, I share a page from Dr. Richard Tan’s original case notes—a tradition inspired by my dear friend Mark Montoya, whose dedication helped preserve and spread the Balance Method throughout our community. ❤️

This week’s case is from May 2013, involving a patient with left groin and hip pain. In this brief note, Dr. Tan documented a fascinating treatment strategy using Image and Mirror. He used the shoulder as a positive image of the area in pain, but also applied the concept to reveersely mirror the lower abdominal/groin region. It’s a beautiful example of how to apply the Balance Method in unexpected and creative ways. Reading his notes today, I’m still amazed by his strategy and point combination. Seeing how a master applies a simple principle is what makes you truly understand its depth.
