A favorite memory from Fiji. 🇫🇯

A few years ago, I had the privilege of traveling with an incredible team of surgeons, nurses, and volunteers to provide ophthalmic and oculofacial reconstructive surgery for patients who otherwise had little or no access to specialized eye care. It was a humbling reminder of how profoundly restoring vision and function can change a person’s life.

Pictured here with Siwa, the head groundskeeper, whose warmth, kindness, and infectious smile made all of us feel right at home throughout our stay. It’s people like Siwa who make Fiji every bit as memorable as its breathtaking beauty.

Grateful for the friendships, the patients who entrusted us with their care, and the opportunity to serve. Looking forward to my next visit to Fiji and the chance to give back once again.

Some friendships have a way of coming full circle.

It was a pleasure for Dr. Michel Choueiri and me to reconnect with our good friend, plastic surgeon Dr. Ahmad Saad, here in Barcelona. The three of us have known each other since our undergraduate days, and years later we all completed parts of our medical training at UCSD.

Why I Stopped Doing Under Eye Fillers and Why You Should Too

The under-eye area is one of the most delicate regions of the face. Over time, I started seeing patterns that made me change my approach. Red flags I frequently saw with tear trough filler include:

❌ Persistent swelling months (or years) later
❌ A bluish or gray tint under the eyes (Tyndall effect)
❌ Puffiness that looks worse in the morning
❌ Filler migration into the lower lid or cheek
❌ Distortion of the natural eyelid–cheek junction

Hyaluronic acid filler attracts water. In the under-eye area, that can mean chronic edema and heaviness instead of true rejuvenation. Many patients who come to see me for “bags” actually have retained filler.

The lower eyelid isn’t just a hollow to fill, it’s intricate surgical anatomy. When volume is needed, it should be restored precisely, in the right layer, and often repositioned rather than simply filled.

Still replaying this incredible evening. I had the pleasure of meeting Ludovico Einaudi after his performance this past weekend. His music was nothing short of transcendent, moving, atmospheric, and deeply human.

How to Tell if You Might Benefit from a Brow Lift

Sometimes it’s not just the eyelids; the brow can play a big role too. Signs the brow may be contributing include:

✅ A flattened brow arch
✅ Your brow sits below the bony rim of the forehead, especially on the outer corner (lateral hooding)

The first set of photos shows before and after healing. The eyes look brighter, the under-eye hollowing is softened, and the face looks more rested overall.

The last set shows the journey just one week after surgery. Still a little swelling and bruising, but already a huge difference. Most patients are back to their routines quickly and love seeing the early transformation.

This approach focuses on repositioning your own eyelid fat, not removing too much, so the result looks smooth, youthful, and natural, not hollow or “done.”

3 Key Things I Look For in Patients with Upper Eyelid Droop

Not all “droopy eyelids” are the same. When patients come to me for an evaluation, I focus on 3 specific issues:

1️⃣ Ptosis (true eyelid droop) – This happens when the eyelid muscle is weak or stretched, causing the lid itself to sit too low over the eye.

2️⃣ Dermatochalasis (excess eyelid skin) – Loose or redundant skin that hangs over the crease, sometimes resting on the lashes and creating heaviness.

3️⃣ Brow Ptosis (low-set eyebrows) – When the brows drop, they push skin down onto the eyelids, making them look even heavier.

Why does this matter? Because treatment isn’t one-size-fits-all. Some patients need eyelid surgery, others benefit from brow lifting, and often it’s a combination to achieve the most natural result.

The key is knowing which factor is causing the droop, and addressing the right one.
