What is a browpexy, and why do I frequently combine it with upper eyelid blepharoplasty?

As we age, the outer portion of the brow — especially the brow tail — can gradually descend. That drop can add heaviness and hooding over the outer upper eyelid.

An upper blepharoplasty removes excess eyelid skin, while a lateral browpexy provides gentle support to the tail of the brow.

For appropriately selected patients, combining the two can improve outer eyelid hooding, symmetry, and overall balance without creating an overly lifted appearance.

It’s a subtle procedure, but sometimes addressing the brow is an important part of creating the most natural upper eyelid result.

Can upper eyelid blepharoplasty be covered by insurance?

This is one of the questions I hear most often — and in some cases, the answer is yes.

When excess upper eyelid skin becomes heavy enough to block part of your vision or interfere with activities like reading or driving, upper blepharoplasty may be considered medically necessary.

Coverage often depends on your symptoms, eyelid examination, photographs, visual field testing, and the specific requirements of your insurance plan.

If the procedure is being performed for cosmetic improvement alone, it generally will not be covered.

This patient came in concerned about vision obstruction and a gradual drop in the outer portion of her brows.

After evaluating her eyelids and brow position, we recommended upper blepharoplasty with lateral browpexy. The blepharoplasty addressed excess upper eyelid skin, while the browpexy provided gentle support to the tail of the brow.

Why has upper eyelid blepharoplasty become so popular?

Upper blepharoplasty removes excess skin from the upper eyelids to create a more open and refreshed appearance.

For the right patient, it can improve hooded eyelids, asymmetry, a tired appearance, and even the tendency to constantly raise the eyebrows to help lift heavy lids.

The goal is not to change the shape of the eyes. A thoughtfully planned blepharoplasty should preserve your natural anatomy and help you look more rested, not overdone.

In some patients, excess eyelid skin can also interfere with peripheral vision, making the procedure functional rather than purely cosmetic.

Lower Eyelid Blepharoplasty in San Diego!

This patient in her 20s came to our San Diego office concerned about genetic dark circles and under-eye hollows.

After examining her anatomy, we recommended lower eyelid blepharoplasty with fat repositioning. Instead of simply removing the under-eye fat, it was repositioned into the tear trough and upper cheek hollows to create a smoother transition from the lower eyelid to the cheek.

She is pictured here just 1 month after surgery.

Dark circles can have many causes — including pigmentation, thin skin, blood vessels, and underlying facial anatomy — which is why eye creams don’t work for everyone.

Beach Talks Ep. 1: What’s the difference between an upper eyelid blepharoplasty and double eyelid surgery?

These two procedures are often confused, but they have different goals!

An upper eyelid blepharoplasty is typically performed to remove or reposition excess upper eyelid skin to create a more open, refreshed appearance.

Double eyelid surgery focuses on creating or defining an upper eyelid crease, most often in patients who naturally have little or no visible crease.

The right procedure depends on your anatomy and what you’re hoping to change.

Before & After Upper and Lower Blepharoplasty in San Diego!

This 49-year-old patient came to our San Diego office looking for a total eyelid refresh.

On examination, she had excess upper eyelid skin, a droopy upper eyelid position (ptosis), and lower eyelid puffiness. After discussing her goals, she decided to move forward with an upper eyelid blepharoplasty, conservative scarless ptosis repair, and lower eyelid blepharoplasty with fat repositioning and a TCA chemical peel to the lower eyelids.

Here she is just 2 months after surgery, and she is thrilled with her results!

Droopy eyelids can be more than a cosmetic concern.

This active woman in her 70s came in because reading had become increasingly difficult. Her upper eyelids felt heavier over time, and eventually they began interfering with one of her favorite activities.

On exam, she had bilateral upper eyelid ptosis, meaning the eyelids were sitting too low and blocking part of her vision.

After visual field testing, she underwent a short in-office procedure to tighten the eyelid lifting muscle through a scarless internal approach.

She is pictured here before surgery and 4 months afterward — now with improved eyelid height and back to enjoying reading again!

Patients ask me about these procedures all the time, so here’s my take as an oculoplastic surgeon!

Brow lifts can be a great option when low brow position is contributing to upper eyelid heaviness.

I’m less enthusiastic about “fox eye” surgery because I prefer procedures that restore and enhance natural anatomy rather than chase a trend.

I’m also cautious with buccal fat removal because removing lower facial volume can make some patients look more hollow as they age.

Under-eye filler can work beautifully in the right patient, but it can worsen puffiness or festoons in others.

I’m not a big fan of facial threads because puckering and migration can occur.

Two treatments I use often? Under-eye chemical peels for fine lines and texture, and blepharoplasty for upper eyelid heaviness and lower eyelid bags.

The best procedure is the one that makes sense for your anatomy and long-term goals.
