A breast cancer diagnosis can bring urgent decisions, but future fertility deserves a place in the conversation, too.

Some cancer treatments may affect ovarian function or reduce ovarian reserve. For patients who hope to have biological children in the future, fertility preservation should ideally be discussed as early as feasible after diagnosis and before potentially fertility-impacting treatment begins.

Depending on the individual situation, egg freezing or embryo freezing may be considered. Modern fertility-preservation protocols may also allow ovarian stimulation to begin at different points in the menstrual cycle when clinically appropriate.

There is no single approach for every patient. Age, ovarian reserve, cancer diagnosis, treatment plan, timing and future family-building goals all matter.

Cancer treatment remains the priority. Fertility preservation is about making sure future reproductive possibilities are considered within that care whenever medically feasible.

Completing breast cancer treatment can open a new chapter, and for some survivors, that chapter includes questions about fertility and family building.

Cancer treatment can affect ovarian function and ovarian reserve, but fertility after breast cancer is highly individual. Some survivors may still be able to use their own eggs, while others may have eggs or embryos frozen before treatment. For some, donor egg IVF may become another family-building option.

Donor eggs may be considered when factors such as significantly diminished ovarian reserve, premature ovarian insufficiency, difficulty obtaining viable eggs, age-related reproductive changes or previous unsuccessful fertility treatment are present. But no single factor automatically determines the pathway.

It is also important to separate two questions: the source of the egg and the ability to carry a pregnancy are not the same thing.

Pregnancy planning after breast cancer should be discussed individually with both the fertility and oncology teams, taking treatment history, current medications, overall health and reproductive goals into account.
Every survivor deserves a family-building plan shaped around her own medical history and future goals.

October is Pregnancy & Infant Loss Awareness Month, a time to acknowledge pregnancies and babies lost too soon, and the families whose lives have been changed by those experiences.

For someone who has experienced more than one pregnancy loss, grief can also come with difficult questions: Why did this happen again? Is there an underlying cause? When should I seek answers?

Recurrent pregnancy loss generally refers to two or more pregnancy losses. Patients do not need to wait for a third miscarriage before discussing further evaluation with a reproductive endocrinologist.

Depending on the individual history, evaluation may consider genetic and chromosomal factors, uterine anatomy, hormonal and metabolic factors, antiphospholipid syndrome, age-related embryo factors and relevant male reproductive factors.

There is no single test or treatment pathway that is right for everyone. The next step begins with understanding the individual reproductive history and identifying which questions are medically meaningful.

Should the donor resemble you? Should she resemble your first child? How important are ancestry, medical history or genetic screening? Will your children share a genetic connection? And what if you hope to have another child later?

There is no universal checklist that determines the “right” donor.For some families, medical and genetic history are the highest priorities. For others, resemblance, ancestry, interests or personality may also carry emotional significance.

The goal is not to create a perfect match or guarantee how a future child will look. It is to understand which characteristics genuinely matter to your family. Future family size can also be part of the conversation. If additional siblings are part of your plans, discussing those goals before beginning treatment may help shape donor selection and embryo planning.
