Dementia Care Aware, LifeLong Medical Care, and the Institute for Healthcare Improvement (IHI) discussed mentation, the 4Ms domain most closely tied to dementia screening, and explored how health systems could use DCA training and resources to strengthen cognitive health assessment and post-screening follow-up.
LifeLong Medical Care, an Age-Friendly certified FQHC, also shared how they deepened dementia screening and follow-up workflows in their Over 60 Health Center through their partnership with Dementia Care Aware.
