If you’re not a Swiftie, feel free to skip this post but do stay around if you’re a therapist interested in developing and strengthening your clinical skills in treating disordered eating and body image.

I know, so many, fellow millennial therapists who have been listening to Taylor Swift since our high school years. I mean “Should’ve Said No” and “Our Song” were 2006 bangers.

Then, as you were trying to cope with college crushes and heartbreak, I know you were blasting the “Flawless” and “Speak Now” CDs from your beat up car. And now, here we are in 2024, listening to TTPD while doing progress notes.

It’s quite the phenomenon to grow up with a musical artist who has captured all of the complicated feelings of being a young woman over the last few decades.

Weight loss is not a panacea to all health issues. Pursuing weight loss can even worsen someone’s health.

✨There are so many intervention points to improving health that don’t involve the intentional pursuit of weight loss. Restrictive diets, weight loss surgeries, and weight loss medications all come with risks to one’s health.

✨ If your client does not want to pursue weight loss in the name of their health and their doctor keeps bringing it up them, here are some things you can consider with them:

1️⃣ Discussing the possibility of changing doctors and meeting with a HAES (health at every size) aligned doctor, these are hard to come by but there are some out there. Check out my “Referrals” Highlight in my IG bio for some referral lists.

2️⃣ Support your client in setting boundaries with their doctor, role play with them in session. “Please do not prescribe weight loss for me. If I’m interested in pursuing weight loss, I will let you know. I would like to know what testing and follow-up treatment recommendations you would provide that don’t include intentional weight loss.”

3️⃣ Help them create a written medical summary to share it with their doctor. The medical summary can include: current medications, allergies, previous surgeries and hospitalizations, previous negative reactions to medications/surgeries, list of current symptoms and history of those symptoms, list of current diagnoses, and family history of medical conditions. Then ask, “Based on this summary and my most recent lab + physical results, what diagnoses, testing, and follow-up recommendations come to mind? I would like to know all potential issues and options please.”

📝 Today, as ED awareness comes to an end, I want to take a moment to remind you: EDs are not just an issue for the rich, white, and privileged.

It’s easy to get caught up in the stereotypes about EDs. But let me tell you something from an ED therapist and clinical supervisor: EDs affect anyone and everyone.

Still don’t believe me? Let’s take a look at some of the research.

🗣 Being a civilian during wartime increases the risk of developing an ED

🗣 Being in the military and experiencing military SA increases the risk of developing an ED

🗣 Experiencing food insecurity increases your risk of developing an ED

We need to dismantle the stereotypes and myths about disordered eating.

EDs have the second highest mortality rate for any mental health issue after opioid addiction.

EDs affect people of any age, gender, race, ethnicity, ability, and socioeconomic status.

It’s not your fault you don’t like your body and desire weight loss.

Your beliefs and ideas about shape, weight, and appearance are influenced by the cultures you are embedded in as well as your time in history.

You can pause and notice when you have a moment of weight or appearance bias and choose to think about acknowledging the diversity and variety of all bodies without judgment. Not one being morally better than another, not one being healthier than another, solely noting bodies come in all shapes, abilities, and sizes and all are deserving of care, dignity, and respect.

Many people aren’t aware that the idea of smaller bodies being preferable to larger bodies is rooted in racism and anti-Blackness.

“By the middle of the 18th century, a lot of French philosophers in particular were arguing that when we’re in the colonies, we’re noticing that Africans are sensuous. They love s*x, and they love food. And for this reason, they tend to be too fat. Europeans, we have rational self-control. This is what makes us the premier race of the world. So in terms of body size, we should be slender, and we should watch what we eat.” As a result, “eating and body size became two of the characteristics that were being used to suggest that these are people who do not deserve freedom.” -Sabrina Strings, PhD

❔Have you ever been on a no or low carb diet? Pretty much everyone has at this point so it’s not your fault if you are terrified of them or feel immense guilt or anxiety after eating them.

👯‍♀️ However, carbs are your friend because they:

— stabilize your blood glucose levels
— provide fuel for your muscles and your brain
— prevent you from feeling tiredness, fatigue, dizziness, irritability
— curb binge eating particularly at mid-afternoon (around 3-4 pm) when blood glucose levels naturally drop and cravings commonly kick in

Are you stuck in this cycle? One of the maintaining factors of binge eating is dieting, restrained eating, and food insecurity.

Not having access to food or not allowing yourself access to food will make you more preoccupied with food and will make it more likely you will binge when you do have access to food.

Binge eating is usually also caused by:

-Malnourishment

-Breaking a food/dietary rule

-Being disinhibited by drugs or alcohol

-Your mood: feeling tired, bored, irritable, lonely, shame, anxious

-Activating situations: a fight, breakup, work stress, etc.

Save these reframes for when you need them ⬇️

Lie: "I'm not that sick" vs. Fact: If you have eating disorder symptoms and you’re in distress and suffering, you are sick enough to deserve acknowledgement, support, + care.

Lie: "I'm never going to get better" vs. Fact: Everyone has the capacity to evolve. What would one small step in the better direction look like?

Lie: "Nobody likes me, I'm worthless" vs. Fact: It’s highly unlikely nobody likes you. There is at least one person that likes you. Your worth is intrinsic and innate. Your value has no because.

Lie: "I will be happy + feel better when I lose more weight" vs. Fact: There is no body or weight that safeguards happiness. You can start feeling happier and better now, in whatever body you are in. Your body is waiting for you to live fully in it.

Lie: "I can't ever eat this again" vs. Fact: You can eat whatever you want, whenever you want (unless of course it’s an allergy or medically required). You do not need to be dictated and controlled by food rules. All foods can fit into your life. A diet of restriction will only continue to make you feel miserable.
