Medic serves as the technical steward and a core contributor to the Community Health Toolkit (CHT), an open-source platform that supports health workers as they provide essential care in their own communities. ed across more than 14 countries, 32 cities, and all time zones. We create resources, build documentation, and offer support to a growing community of partners and builders working to improve health in the hardest-to-reach communities. Medic is a US 501(c)(3) public charity with tax-exempt status.

This is Simon Pierre Gabo Gahou.

Since 2018, he has cycled through Gagnoa, south-central Côte d’Ivoire, caring for 60 households as a community health worker (CHW).

He kept track of every family, every pregnancy, and every child on paper.

That is until a Community Health Toolkit-powered app called SanteComCIV changed everything.

Now, Simon Pierre has a tool that guides disease diagnosis and referral, flags who needs urgent care, and gives recognition and visibility to those like him on the frontlines of care.

“Everything is quicker with the application,” he says.

The system is now deployed across nearly half the country. Over 8,200 CHWs like Simon Pierre use it to deliver quality care to 6.3 million people.

And the results tell an incredible story: 94% of supervisors say that the tool supports better clinical decision-making, and over 90% of households reported increased use of health services.

This is what transformative tech looks like when it’s developed with the communities it serves.

When communities shape the technology they use, health systems grow stronger.

That’s the core belief behind the CHT Community of Practice. And, in 2025, it was affirmed in amazing ways.

We welcomed 262 new members—a remarkable 344% increase from the previous year.

Contributions doubled, connections between members grew fivefold. A challenge in Kenya fuelled progress in Mali and a breakthrough in Nepal drove change in Jamaica.

This is the power of community: a global ecosystem driving transformation, together.

That’s how many all-time moments of care have been delivered by community health workers through the Community Health Toolkit.

In 2025 alone, they delivered over 75 million caring activities, from pregnancy registrations to malnutrition screenings; immunization checks to hypertension assessments.

That’s what a platform built for scale, at the last mile, supported by a global community, can do.

We’re proud to share our 2025 Annual Report: Rooted in Community, Built to Last.

2025 wasn’t a quiet year. For anyone in the global health sector. Funding shocks, a rapidly shifting aid landscape, the accelerating pace of AI…all brought challenges and opportunity.

Yet, through it all, something extraordinary happened: our community came together, not apart.

By the end of 2025, the Community Health Toolkit supported 182,636 health workers across 24 countries, reaching an estimated 90 million people. Eight national governments now rely on the CHT to power their community health systems.

This report belongs to every health worker, every partner, and every supporter who made that possible.

Maureen faced something no mother should.

Yet she chose to channel her experience into helping others, becoming a Community Health Promoter supported by Lwala Community Alliance.

Thanks to The Ministry of Health Kenya's steadfast dedication to the digital transformation of their community health program, Maureen is salaried, skilled, supplied, supervised, and supported with a digital health system that reminds her of household visits and guides her while she's there.

She also now advocates for other CHWs around the world to receive the same.
