The CDC reports that three in four U.S. adults have at least one chronic condition, and more than half have two or more.

Celebrating Yim’s successful recovery. 💙

After a fall that caused a fracture, Yim came to Lawton Skilled Nursing & Rehabilitation Center in San Francisco, CA, needing support with walking and daily activities.

With guidance from the rehab team, she worked through a personalized recovery plan focused on safe movement, strength, and independence. Her care team also completed a home assessment to help support a safe discharge plan.

Yim made meaningful progress and was able to return home safely.
