Ilioinguinal neuralgia: when pelvic nerve pain isn’t pudendal neuralgia 👇

Burning, shooting, or sharp pain in the groin or around the p***c bone can sometimes point to irritation of the ilioinguinal nerve.

Because the ilioinguinal and pudendal nerves can both cause pain in the l***a or p***s, it’s easy to assume the pudendal nerve is responsible. But location matters. One important distinction? Ilioinguinal neuralgia can involve pain along the inguinal canal/groin, an area the pudendal nerve does not innervate.

🔹 What does it feel like?
Ilioinguinal neuralgia may cause burning, shooting, or sharp pain along the groin, at the top of the p***c bone, and sometimes at the top of the l***a or base of the p***s.

🔹 What can cause it?
Potential causes include:
• Inguinal hernia or hernia repair
• P***c symphysis trauma or separation
• Surgical trauma
• Scar tissue, including from C-section scars

🔹 How is it treated?
Treatment depends on the underlying cause and may include pelvic floor physical therapy, medications, nerve blocks, addressing an existing hernia, or—in select cases—surgical intervention.

And remember: pelvic nerve pain isn't always pudendal neuralgia. Several nerves can contribute to pelvic and ge***al pain, and it’s possible to have more than one neuralgia at the same time. A thorough clinical history and physical examination can help identify which nerve(s) may be involved.

Pelvic pain is one of the more challenging pain diagnoses in medicine 🧑‍⚕️

It’s often referred to as an “invisible pain diagnosis” because people with pelvic pain don’t LOOK like they have pain, often they look healthy. There’s no obvious wound or injury 🩹
