Nobody tells you that healing is supposed to hurt.

Not as a punishment. Not because something is wrong with you. But because working through trauma means going toward the thing your entire nervous system has been organized around avoiding.

It's painful. It's distressful. There will be sleepless nights. You will have to build resilience you didn't know you needed.

But you get to choose. Stay in the darkness of unprocessed trauma — or walk through the pain of healing toward something that actually has hope on the other side.

Dr. Mike and Sarah Burney, LCSW on what the path of trauma recovery actually looks like.

New research just dropped in The Lancet (April 2026) — GLP-1 medications (like semaglutide) are showing real promise for Alcohol Use Disorder.
Dr. Mike Mah breaks down the latest randomized trial: what it actually found, who it might help most, and why we’re not rushing to call this a miracle cure yet.
Swipe through for:
• The current treatment gap in AUD
• How GLP-1s work on brain reward pathways
• Key results from the new trial
• Important limitations & clinical context
This is early but meaningful evidence. Promising — especially for patients with AUD + obesity — but not first-line yet.

Your brain can actually rewire after trauma. This is not a metaphor.

The changes PTSD makes to your nervous system — the hypervigilance, the reactivity, the flashbacks — are real. And they are reversible.

It takes time. It takes courage. It won't happen overnight. But neuroplasticity is real, the evidence is there, and healing is possible.

Dr. Mike and Sarah Burney, LCSW on what recovery from trauma actually looks like — and why there is genuine reason for hope.

Narcissism isn't arrogance. It's armor.

When a child learns early that love is conditional and abandonment is always possible, they adapt. Some shut down. Some get high. Some become relentlessly successful. And some build a wall so impenetrable that no one can ever confirm what they already secretly believe — that there's something deeply wrong with them.

That's not grandiosity. That's a terrified kid who never got what they needed.

Dr. Mike and Sarah Burney, LCSW on how complex trauma quietly shapes adult personality.

Trauma isn't what happened to you. It's the wound it left behind.

😣 Taking Trazodone for sleep but waking up with a stuffy nose or sinus pressure?
Dr. Mike Mah here — this is surprisingly common.
Trazodone blocks alpha-1 receptors, which causes blood vessels in your nose to dilate. That leads to congestion and sinus pressure for many people.
It’s not allergies or a cold — it’s a side effect of the medication.
If you’re experiencing this, don’t just push through it. Talk to your psychiatrist — a dose adjustment or switch can often fix it quickly.
Drop a 👃 if this is you, or comment “TRAZODONE” for more sleep med tips.
Always consult your doctor before changing any medication.

😣 Struggling with brain zaps, headaches, or feeling emotionally all over the place when trying to get off Pristiq, Effexor, or Cymbalta?
Dr. Mike Mah here with a practical tip that helps many patients.
SNRIs like Pristiq and Effexor have a short half-life, so blood levels drop fast and discontinuation symptoms hit hard — even with a slow taper.
One effective strategy: About a week before stopping the SNRI, start Prozac (fluoxetine) 20 mg. Then discontinue the SNRI. Stay on Prozac for another 7–14 days before tapering off.
Because Prozac leaves the system very slowly, it creates a gentler landing and softens the withdrawal.
Note: At low doses, these SNRIs mostly affect serotonin anyway. This bridge isn’t perfect for everyone, but it’s often much easier than stopping cold.
Always work with your psychiatrist — never change medications without guidance.
Drop a 🧠 if this is helpful, or comment “BRIDGE” for more tips.

Curious about Buspirone (formerly Buspar)? One of the most misunderstood anxiety medications — but it has a unique place in psychiatry.
Dr. Mike Mah breaks it down in plain language:
✅ How it actually works in the brain
✅ Why it’s non-sedating and non-addictive
✅ When psychiatrists typically prescribe it
✅ Dosing, side effects, and realistic expectations
Swipe through for a clear, no-hype education on this gentle anxiety option.
Not a fast-acting “rescue” med like benzos — but for the right patient with ongoing generalized anxiety, it can be genuinely helpful.
Comment “BUSPIRONE” if you want more tips or have questions 👇
Always discuss with your psychiatrist before starting or changing any medication.
