Our organization's mission is to improve the health and well-being of our community.

The Center for Well-Being is a small, high-impact non-profit that helps individuals and families live healthier lives through nutrition, health education and systems change. Our mission: To promote the well-being of the whole person by empowering people with the knowledge, skills, and spirit to take responsibility for personal, family, and community health. We offer a menu of health promotion classes covered by most insurance plans as well as place-based programming at school sites, work places and other community environments. Priority areas are diabetes, heart disease, healthy nutrition and physical activity promotion, pain management and tobacco control.

⚠Starting January 1, 2027. California will ban the manufacturing and importing of battery-powered, single use ni****ne vapes. Then, starting January 1, 2028, retail sales of these products will also be banned in California.

♻The goal is to reduce vaping-related harm and environmental damage. Disposable vapes contain batteries and other materials that can cause fires in landfills and recycling facilities and may release toxic chemicals.

Today we are celebrating Rosa's 7-year anniversary! 🎉As our Clinical Services Supervisor, Rosa plays an important role in supporting both our Nutrition and HearWorks programs. From helping patients connect with our services to overseeing referrals, scheduling, and supporting our team with daily operations, she helps ensure our patients receive the care and support they need. We are so grateful to have her as part of the Center for Well-Being family! Happy 7-year anniversary, Rosa! We truly appreciate you! ❤️

Huge congratulations to Ariel Muirhead, our Clinical Integration Manager and Bilingual Registered Dietitian, on celebrating her 7th work anniversary!

For seven years, Ariel has been a positive force and an invaluable leader at the Center. She brings a vibrant, creative energy to everything she does and constantly goes above and beyond to support our community.
She makes incredible impact in:
Patient Care: She ensures both English and Spanish-speaking patients receive the whole person quality care they need.
Leadership & Mentorship: She manages our care coordinators and provides mentorship to our Dietetic interns.
Community Wellness: She leads our diabetes programs, exercise classes and nutrition education programs with innovation and dedication.
Ariel, your intelligence, compassion, and commitment empower our patients and inspire our entire team daily. Thank you for 7 years! 💚

Recovering from a heart event can bring a lot of emotions. At HeartWorks, we care about how our patients feel, along with how they’re doing physically. ❤️

One outcome we’re proud to share is an average 72% improvement in our patients’ PHQ-9 scores—a questionnaire used to measure symptoms of depression.

We track this because emotional well-being is an important part of recovery. Through cardiac rehab, our team provides encouragement, connection, and support every step of the way.

Our team had the opportunity to attend the Sonoma County Perinatal Care Summit hosted by Partnership HealthPlan of California last Friday.

It was a great afternoon of learning, connection, and collaboration with organizations across Sonoma County that are working to better support pregnant individuals and families throughout pregnancy, birth, postpartum, and beyond.

As a nutrition leader in our community, the Center for Well-Being is proud to support this population through nutrition education, individualized counseling, and resources that help families build healthy habits during pregnancy and throughout the postpartum period.

We’re grateful to be part of these important conversations and to continue strengthening partnerships that improve care, access, and support for families in our community. 💙

Today we’re celebrating 6 years with our amazing HeartWorks Senior Manager, Erin Roosen! ❤️

Erin joined us during the pandemic and has been such an important part of rebuilding and growing our HeartWorks cardiac rehab program into what it is today. She leads with so much heart and always puts our patients first.

Erin truly cares about every person who walks through our doors and is always focused on providing the highest quality care possible. Her dedication, compassion, and leadership have made such a difference for our patients, our team, and our community.

Our patients love her, our team loves her, and we are so lucky to have her at the Center for Well-Being.

Celebrating 19 amazing years with Jeanette!

Jeanette Magana joined CWB in 2007 as a front office assistant and now leads our entire clinical team as Director of Clinical Operations. She is passionate, creative, and has a solution for everything. Most of all, Jeanette brings heart, love, and kindness to our team, our patients, and our mission. She is truly woven into the fabric of who we are at CWB!
