Yesterday we published a longer piece on trauma care in Sarasota — what evidence-based trauma treatment actually involves, and what to expect if you are considering it for the first time.

A few things worth saying clearly. Evidence-based trauma therapy is structured. It is not open-ended weekly venting and it is not reliving the worst moments on repeat. A doctoral-level psychologist will spend the first sessions building a careful map — what happened, what the symptoms are now, what supports you have in place, what coping skills are already working. The active processing work comes after that foundation, at a pace your nervous system can tolerate.

Approaches with strong research support include trauma-focused cognitive behavioral therapy, prolonged exposure, cognitive processing therapy, and adaptations of these for complex and developmental trauma. The right approach depends on the person, the type of trauma, the time elapsed, and what else is going on in life right now.

What good trauma care is not: pressuring someone to disclose more than they are ready to, treating the trauma in isolation from the rest of their life, or promising a number of sessions in advance. Healing has a shape but not a fixed schedule.

A quick reminder for our Venice neighbors. Our Venice office offers Saturday appointments — which we know matters for families whose weekdays are already stretched thin between work, school pickup, and the small administrative load that comes with parenting in 2026.

Saturdays often work well for first appointments. There is a little more room to breathe before and after. Parents who want a child seen can come together without one parent missing a workday. Couples who want to start counseling together can do so without negotiating two workplace calendars on a Tuesday afternoon.

"Just get over it" assumes the brain works like a filing cabinet — pick up the bad memory, move it to the back, close the drawer. The neuroscience does not support that picture.

Research from the National Institute of Mental Health and decades of clinical study show that traumatic memories are stored differently than ordinary memories. Under high threat, the parts of the brain that organize experience into narrative — putting events in order, attaching language, marking the memory as past — go partially offline. The parts that encode sensory and emotional intensity stay fully online. The result is a memory that does not feel finished. It can be triggered by a smell, a tone of voice, a particular light through a window, and the body responds as if the threat is current.

This is why telling a trauma survivor to "move on" tends to backfire. The intent is kind. The instruction asks the cortex to manage something the cortex was not online for in the first place.

Evidence-based trauma treatment works the way the brain actually works — gradually, in a safe relationship, with attention to the body as well as the story. It is not fast and it is not magic. It is durable. People who do this work often describe it not as forgetting, but as the memory finally taking its place in the past.

One of the quieter shifts we see in couples work happens when one partner starts individual therapy.

Not because the relationship is the problem. Often the relationship is fine and the individual therapy is about something else entirely — a stalled grief, a long-running anxiety pattern, a work situation, an old family-of-origin wound. But when one partner starts looking at their own patterns with a psychologist, the relationship usually shifts too. The reactive flare-ups soften. The familiar argument starts taking a different shape. The partner who isn't in therapy often says, unprompted, that something feels different.

This isn't magic. It's that most relationship friction is downstream of each person's individual nervous system, individual history, and individual unspoken assumptions. Couples counseling addresses the system between two people. Individual therapy addresses what each person is bringing in. The two complement each other and often a season of one makes the other more productive.

A common pattern in our practice: a person comes in for individual work, finds it useful, and a few months later their partner asks for a referral. Sometimes it ends there. Sometimes the couple eventually decides to do a stretch of couples work together. Either path is legitimate.

A question for the community today. If you could go back and tell your younger self one thing about mental health — one thing you wish you had known at twenty, or fifteen, or eight — what would it be?

Maybe it's that asking for help isn't weakness. Maybe it's that the thing you thought was just your personality had a name and was treatable. Maybe it's that the people who told you to toughen up were carrying their own untreated pain.
