You’re home alone and having a heart attack. What do you do?

1. Call 911
2. Unlock / Open Your Door
3. Sit Down or Lie Down
4. Notify a Friend/Family Member

Chew a single non–enteric-coated aspirin (162–325 mg) while waiting for help, unless there is a known aspirin allergy, active bleeding, or a clinician has told the patient not to take it.

Do optimists really live longer? Yes, and the science is interesting.

Studies following hundreds of thousands of people found that optimists live, on average, about 15% longer and are far more likely to reach 90+.

But new research suggests it’s not really about being optimistic — it’s about NOT being pessimistic.

3 reasons why:

1. Less chronic stress. Optimists (and non-pessimists) don’t marinate in worst-case scenarios.

2. Faster recovery from setbacks. Adversity is inevitable, but it’s how fast you bounce back. Quicker recovery = less toll on your heart and immune system over time.

3. Healthier behaviors. People who expect good outcomes are more likely to actually show up for their health: moving, eating well, and sticking with treatment.

Now, this data is observational and evolving, but the science supports optimism.

I leave you with this quote from Alex Hormozi (Alex Hormozi): “If you can be in a bad mood for no reason, you might as well be in a good mood for no reason.”

Does a calcium score of zero mean you won’t have a heart attack?

The short answer is no, and here’s why:

The goal of any screening test is to prevent future disease in patients without symptoms.

A calcium-scoring CT measures calcium, or the hard plaque, in the coronary arteries.

Hard plaque forms from soft plaque that matures over time.

However, 80% of heart attacks are from soft plaque rupturing.

A calcium scoring CT scan doesn’t detect soft plaque.

For patients under 40 with a score of 0 is much less reassuring because soft plaque may not have matured yet.

Although calcium scoring may help in certain situations, it rarely should change management.

Risk factors like smoking, diabetes, and high ApoB need to be addressed irrespective of the score.

A calcium-scoring CT scan tells only part of the story.

So no - a calcium score of 0 means doesn’t mean you are safe from a heart attack.

Strength is a long-term investment.

The deadlifts at 30 help you pick up your suitcase at 80.

How to lower your blood pressuređŸ«€âš ïž

The most powerful lifestyle levers are diet, exercise, and weight loss.

A diet rich in fruits, vegetables, and lean meats, and limiting sodium-rich processed foods.

Exercise. The heart is just a muscle, and exercise trains the heart to be more efficient with each beat.

Weight loss. Losing excess body fat reduces the workload your heart has to supply. 

If you snore, stop breathing at night, and wake up unrested, get evaluated for sleep apnea. It’s one of the most common reasons pressure won’t come down.

Medication is not a failure. Genetics and arterial stiffening are real, and no amount of training changes your arteries as you age.

There is a wide range of effective blood pressure medications that essentially do one of three things

Relax blood vessels, remove fluid from the system, or slow the heart. 

Check blood pressure at home.
The current optimized goal from the SPRINT Trial is 120/80.

Know your numbers.

A new drug lowered Lp(a) by up to 80%, but It didn’t lower heart attacks

Lp(a) is an LDL-like particle with an extra protein tail. Stickier, more inflammatory, more likely to drive clot formation.

It’s 90% genetic One in five people worldwide has an elevated level. Closer to a third of people with premature heart disease do.
Novartis enrolled 8,323 patients with established cardiovascular disease and elevated Lp(a) levels. Half received monthly injections the other half placebo for four years.

The drug crushed Lpa levels. The number of events didn’t move.
Does this mean Lp(a) doesn’t matter? Not necessarily.

The genetic evidence from Mendelian Ramdomizations that Lp(a) is an independent cause of heart disease isn’t going anywhere.

Three explanations why this happened:
1. Decades of plaque can’t be overturned in a few years of treatment

2. The benefit was washed out due to the patients being on optimize lipid therapy

3. Pelacarsen’s mechanism or depth of Lp(a) reduction may simply be insufficient

Two different medications trials are running now with results pending.

A hypothesis got tested properly and didn’t hold.
That’s not a failure that’s medical science working.

The best way to measure your blood pressuređŸ«€

7-2-2 Rule:

7 days take your blood pressure.
2 times per day, morning and evening.
2 measurements each time.

Then take the average of all recordings.

Don’t solely rely on one annual blood pressure reading.
