Happy Friday! As this chapter of Shelby Pediatrics closes, we want to thank all of our wonderful patients and families for allowing us to be a part of your lives. It has been an honor to watch your little ones grow, learn, laugh, and thrive. We will cherish the memories, the smiles, and all the special moments we’ve shared.
Thank you for trusting us with your children. You will always hold a special place in our hearts. We will be open Monday 8am-5pm one last time. If you need anything please don't hesitate to reach out :)

The office doors will be officially CLOSED starting Tuesday September 1'st. Through the month of September you will be able to call the office phone 502-633-7337 from the hours of 9am-1pm if you need to request records to be emailed/faxed. If you have not chosen a new provider yet that is okay, someone will still be able to fax all records once you choose one. We suggest you go through your new provider and have them fax us a records release form. We can also email records directly to you, we would just need you to call the office phone. Lastly, if you are interested in the direct patient care please call our office to be sure that you are on our list so that we can reach out directly to you to keep you updated on everything that is to come. Again, we appreciate everyone's patience through this transition. If you have any questions please don't hesitate to call our office. Thank you!

We just wanted to remind everyone that our last day open is next Monday August 31'st. We do still have open appointments until then so please don't hesitate to call us and schedule! If you are needing records we do ask that you give us a 24 hour notice if you would like to pick them up or emailed. We also suggest that once you find a new provider they can fax over a records request and we can fax records directly to them. If you have any questions please call our office directly 502-633-7337.
Also, there is not really an update for the Direct Patient Care. Dr. Siegel is still trying to find a space and figure out the details that require getting it started. As of right now he is taking names of people who are truly interested and then once things are set in stone or there is a plan to move forward someone will reach out one by one. We apologize for the delay in the update, it has been a challenging journey trying to figure everything out and making it a smooth transition for all of our patients. We appreciate everyone's patience and promise to help in any way we can to make sure you get your child's records!

We would like to thank everyone for the love, support, and patience over the past several weeks. I know everyone has been asking for an update so we will post something later this week or early next week on what Dr.Siegel has planned with the Direct Patient care. There has also been a lot of questions and concerns about medical records. You can call and request to pick them up or have your new provider send us a records request. Be sure to call and check with our office to make sure there is no outstanding balance on the account as we can NOT send over records until the account is paid off. We are offering payment arrangements to make it easier so please call our office 502-633-7337 so we can help you set something up. We do have a stack of current requests so we do ask that you give Grace as we are working very had to get those out as fast as we can. Also, we are seeing patients until the end of August so if you need well checks or school physicals call our office and we will get you in before then. We are here to help guide you through this transition so please don't hesitate to call our office if you have any questions about what to do to transfer or about the Direct Patient Care. We hope everyone knows how grateful we are for each and every family who has supported us over the years. Have a blessed day and rest of your week! :)

We are beyond thankful for the support we have received through the announcement. If you are truly interested in the DPC model please call our office 502-633-7337 so we can add you to our list! Unfortunately, if your child has Medicaid you are not able to join due to a Kentucky law. Once we have a completed list we will keep everyone up to date on what the next plan is! Again, if you have any questions or concerns please don't hesitate to reach out.

Happy Thursday! It is crazy how fast June has flown by. Now is the time to call and get your child's Well check, school Physical and/or sports physical scheduled. We can make sure you have all of the paperwork necessary for school registration. Happy Friday! It's officially summer for most of the students now! Reminder, we are still closed on Saturday's, but with 3 providers through the week we have plenty of availability to get in for physicals. Have a happy, safe, and warm weekend :)
