If you take medications, you may need to adjust your dose or add a second medication. Some psychiatric providers allow their patients to take more ADHD medication in the premenstrual window, when efficacy often decreases.

Although estrogen therapy isn’t proven to help ADHD, we do know that it can improve executive function in the general population.

You may also benefit from what’s called “external scaffolding,” which means moving tasks from your brain into your environment. That could include visual cues (supplements on the counter, dog leash by the door), calendars, or alarms—anything that removes friction between you and the tasks you want to accomplish (or, alternatively, things that add friction to unwanted behaviors).

There are many kinds of trauma, and not all of them are reflected in Adverse Childhood Experiences (ACE) scores. However, when children are exposed to dysfunction within the family — such as divorce, neglect, abuse, or incarceration — they are more likely to develop chronic pain and illness later in life.

This connection is linked to changes in the nervous system, stress response, and inflammation. Even when our circumstances change, the body can continue holding onto hypervigilance, and the brain may begin to interpret pain as a potential threat.

But here’s the good news: this is not destiny. Even if you’ve experienced chronic illness after childhood adversity, your body can learn safety at any age.

From puberty to postpartum to perimenopause, hormonal shifts can affect focus, emotional regulation, motivation, and executive function. Many people notice their ADHD symptoms worsen before their period, after pregnancy, or during major life transitions.

This isn’t “being inconsistent.”
There’s a real biological connection between hormones and the brain pathways involved in ADHD.
