We’re introducing a new medication review policy to help us provide the safest and most accurate care possible.

Please bring an up-to-date list of all medications you take to every appointment, including:

• Prescription medications
• Over-the-counter medications
• Vitamins and supplements
• Herbal products

Keeping your medication list current helps us maintain accurate records and provide the best care for you.

Please note: If you do not bring your medication list, your appointment may need to be rescheduled.

Happy October! We are pleased to announce that Jason Patrick, a board certified Family Nurse Practitioner, has joined our team at Family Health Center of Sierra Vista! Jason has been trained to provide care for patients through all stages of their lives. He worked in the emergency department as a registered nurse for 5 years, until receiving his Master of Science in Nursing from the University of Phoenix as a Certified Family Nurse Practitioner. Jason was raised in Southern California, but has resided in Arizona since 2009. He enjoys many outdoor hobbies and tackling new projects at home. Jason is currently accepting new patients and usually has same day appointments available. We are thrilled to have Jason as part of our team and are confident he will provide you with the excellent care you deserve!
