A GREAT NIGHT AT OYC!

Last night Dr. P had the privilege of speaking with the members of Owasco Yacht Club and sharing our mission, experiences, and stories. It was an incredible evening with a fantastic turnout, great conversations, and the opportunity to personally autograph dozens of books for those who attended.

Thank you to everyone who came out, asked thoughtful questions, and made the evening so enjoyable. The warmth and sense of community at OYC were on full display.

If you’re looking for a place that brings people together, Owasco Yacht Club is truly a special destination—not just for boating enthusiasts, but for families as well. The friendships, activities, and community atmosphere make it a wonderful place to be a member.

THE HARDEST THING TO CONVINCE PATIENTS OF…

One of the hardest things to convince patients of is the importance of being proactive.

Research has shown that patients with one bone marrow lesion (subchondral stress/microfracturing) are 9 times more likely to eventually need a knee replacement if it is not addressed. If there are two or more bone marrow lesions, the risk increases to 13 times more likely.

The challenge? Most of the time these bone marrow lesions cause significant pain, but not always. Some patients have these findings on imaging and feel relatively good. When they’re not hurting, it’s difficult to convince them that action may still be necessary.

Think about it this way: if you were told you had a small area of metastatic cancer, would you wait until it caused symptoms before treating it? Of course not. You’d want to address it as early as possible.

That’s why being proactive matters.

Dr. Hernigou’s research demonstrated that treating these bone marrow lesions with bone marrow aspirate injections resulted in an 82% success rate of patients never needing a knee replacement. The earlier these issues are identified and addressed, the better the opportunity to preserve the knee.

If you’ve been told you have bone marrow lesions (subchondral stress/microfracturing), don’t ignore them simply because you’re having a good day or aren’t currently in pain. At the very least, these findings should be monitored regularly. And if symptoms begin to develop, act on them immediately.

The goal isn’t just to reduce pain today—it’s to protect your knee for the future.

TREAT IT EARLY, STAY OUT OF THE OPERATING ROOM

Peter recently joined Dr. P on The Local Show to share an important lesson about shoulder injuries: timing matters.

When Peter began experiencing left shoulder pain, he sought answers early. Imaging revealed a partial-thickness rotator cuff tear before the tendon completely tore away from the bone. Because it was caught in time, we were able to create a comprehensive treatment plan utilizing bone marrow aspirate cells, PRP injections, laser therapy, and total body fitness to help support his body’s natural healing potential.

The outcome? Peter was able to avoid the path that his right shoulder took years ago that ultimately required a rotator cuff repair surgery.

AMY’S SUCCESS STORY

When Amy first came to Victory In Motion, Home of Knee Repair, NOT Knee Replacement®, she was frustrated and discouraged.

After suffering a meniscus root tear, she underwent a meniscal root repair with another practice and surgeon. Despite going through surgery, she continued to experience pain and was unable to return to the activities she loved most.

JAY HITS A MAJOR MILESTONE!

Success is built one step at a time, and here Jay is celebrating a huge achievement—he has officially worked his way down to 350 pounds!

PARTIAL THICKNESS VS FULL THICKNESS ROTATOR CUFF TEARS

Many people assume a partial rotator cuff tear is “not a big deal” because they can still move their shoulder and function day to day. The problem? Partial thickness tears can continue to worsen over time and eventually progress into full thickness tears.

This is why early intervention matters.

Using ultrasound guidance, the exact location of the partial tear can be visualized in real time so that biologic augmentation including bone marrow aspirate cells (BMAC) and PRP can be injected directly into the damaged tendon tissue. These regenerative medicine procedures are designed to support healing and improve the tendon environment before the tear progresses further.

Research by Dr. Philippe Hernigou demonstrated the powerful effects of biologic augmentation in rotator cuff repair procedures. In the study, patients receiving bone marrow aspirate concentrate (BMAC) during rotator cuff repair showed:
• 100% healing at 6 months compared to 67% without biologic augmentation
• Approximately 87% intact repairs at 10 years compared to 44% in controls

The key takeaway: don’t ignore shoulder pain and partial tears for too long. Once a tendon fully tears and retracts away from the bone, biologic augmentation alone cannot reattach it. Surgery is the only option at that point.
