Adult acne isn’t just teenage acne that forgot to leave. And in women, hormones—and perhaps metabolism—may be playing a much bigger role than we once appreciated. 🔬
As a dermatologist, I see this constantly: women in their 20s, 30s, 40s and beyond suddenly developing or continuing to struggle with inflammatory acne, often concentrated around the chin, jawline and lower face.
And the numbers may surprise you.
📊 In one large U.S. study, acne was reported in approximately:
• 51% of women in their 20s
• 35% in their 30s
• 26% in their 40s
• 15% even after age 50
Adult female acne is therefore anything but uncommon.
Why does it happen?
Acne is still driven by the familiar combination of excess sebum, follicular plugging, Cutibacterium acnes and inflammation—but in adult women, androgen signaling and hormonal sensitivity of the sebaceous gland can be particularly important.
Interestingly, another pathway is receiving increasing attention: insulin resistance.
A newly published 2026 study in the Journal of Integrative Dermatology evaluated 319 women with acne. More than 80% had adult female acne, and investigators found that 99.4% exceeded the study’s HOMA-IR threshold for insulin resistance.
That number deserves attention—but also perspective. This was an observational study without a non-acne control group, and the insulin-resistance cutoff used was intentionally sensitive and relatively nonspecific. So this does not mean that 99% of women with acne are insulin resistant.
What it does raise is a fascinating question:
Could metabolic signaling be contributing to acne in a subset of women?
Insulin and insulin-like growth factor-1 can activate pathways including IGF-1 → PI3K/Akt → mTORC1, while hyperinsulinemia may also decrease s*x hormone-binding globulin and increase biologically available androgen signaling.
Translation: for some women, what is happening metabolically may ultimately be showing up on the skin.
So how do we treat adult hormonal acne?

Treatment still needs to be individualized, but our toolbox can include:
✨ Topical retinoids
✨ Benzoyl peroxide
✨ Azelaic acid
✨ Clascoterone
✨ Oral contraceptives when appropriate
✨ Spironolactone, one of my favorite tools for adult female hormonal acne
✨ Isotretinoin for appropriate resistant or severe disease
And there may eventually be another interesting player:
Metformin.
Metformin improves insulin sensitivity and alters insulin/IGF-1/mTOR signaling. Studies have already suggested improvement in acne—particularly in women with PCOS or demonstrable insulin resistance, and emerging research is examining its role even outside PCOS.
Does that mean every woman with hormonal acne should be placed on metformin?
Absolutely not.
But it does mean that persistent adult female acne may occasionally warrant looking beyond the skin—particularly when accompanied by irregular periods, central weight gain, acanthosis nigricans, elevated fasting insulin, PCOS features or other evidence of metabolic dysfunction.
The future of dermatology is increasingly about understanding that the skin isn’t an isolated organ.
Sometimes acne may be telling us something about hormones.
And sometimes it may be telling us something about metabolism.
That intersection is where things get really interesting. 🤨
