You don't have to choose between your recovery and being a good mom. At Primary Care Partners of South Bend, you can have both. Seeking help is one of the strongest things a mother can do.

Project MAT is our specialized program supporting expecting and current mothers facing opioid and other substance use during pregnancy. It's a judgment-free space built on compassion, confidentiality, and respect.

The program is led by Dr. Brandon Zabukovic, an expert in managing addiction during pregnancy and a dedicated advocate for mothers and their babies. Under his care, you'll receive personalized medical treatment for you and your baby, help building a practical recovery plan, and support with real-life needs like safe housing and reliable transportation.

To learn more or begin your journey, call 574-251-1200 or reach out online. We're here to help you be the best mom you can be.

By the time you feel thirsty, your body is often already running low. Dehydration builds quietly, and its early signs (fatigue, a headache, a cranky kid) are easy to brush off as something else.

This summer, know what to watch for, how to stay ahead of it, and the warning signs that mean it's time to seek care right away. Some signs to watch out for include: dizziness, confusion, dark-colored urine, not urinating for hours, rapid heartbeat, and/or trouble staying awake.

"Dr. Swan is the best doctor I have ever had. I was incredibly scared to start seeing a doctor after many years of not seeing one and not having insurance. She is patient, kind, and listens well. I couldn't ask for anything more."

Coming back to care after time away takes courage, but Dr. Swan meets patients exactly where they are.

Patients consistently describe Dr. Swan the same way: patient, kind, and someone who truly listens. She takes her time, asks thoughtful questions, and answers every one of yours. Whether you're managing a chronic condition or finally reconnecting with regular care, she makes the exam room feel like a place you can be honest. Her passion for preventive care and health literacy means you don't just leave with a plan, you leave understanding it.

At Primary Care Partners of South Bend, patients choose Dr. Swan for care that feels calm, respectful, and genuinely collaborative. She's currently accepting new patients of all ages.

When your child was a baby, you never missed a checkup. But as kids get older, the yearly well-child visit is often the first thing to slip off the calendar.

Here's the thing: it matters just as much at 12 as it did at 12 months. These visits are important for tracking growth, catching both physical and emotional concerns early, and building a relationship with a provider your child trusts.

Read why this visit is worth looking forward to: https://primarycaresb.com/?p=14112&preview_id=14112&preview_nonce=6ebf4759a6&post_format=standard&_thumbnail_id=14127&preview=true
