📢 Important Update: New Appointment Policy at Legacy Eye Care
To continue providing timely, high-quality care to all of our patients, Legacy Eye Care has implemented a new Appointment Cancellation and Late Arrival Policy.
✅ Please provide at least 24 hours' notice if you need to cancel or reschedule your appointment. This allows us to offer the appointment time to another patient in need of care.
💙 We understand that illnesses and family emergencies happen. Late cancellations due to these circumstances will be excluded from this policy.
⚠️ Missed Appointments & Late Cancellations
Failure to provide 24 hours' notice may result in a $50.00 Missed Appointment/No-Show Fee. This fee is not billable to insurance and is the patient's responsibility.
📅 Repeated missed appointments and late cancellations can affect access to care for all patients. Three (3) missed appointments or late cancellations within a 12-month period may result in discontinuation of care with our practice.
⏰ Late Arrival Policy
A 15-minute grace period is allowed for scheduled appointments. Patients arriving more than 15 minutes late will be asked to reschedule.
