Losing weight and losing yourself are not the same thing, but too often they happen together.

In this episode, Jim LaValle, internationally recognized clinical pharmacist, author, and board-certified clinical nutritionist, shared two real patient cases from A4M: 40 pounds lost with only 3 pounds of lean mass. 39 pounds lost with only 2 pounds of lean mass. No side effects.

What’s the difference? Addressing the other factors. This is what GLP-1 therapy is supposed to look like when it's done right.

New breast cancer screening guidance just came out from the American College of Physicians, and it's already controversial. Not for the reason you'd think.

The guidance recommends most average risk women start screening at 50. Critics are pushing back, and hard, because the data shows Black women have a 40% higher mortality risk from breast cancer even with identical screening. Starting earlier, in the 40s, has been shown to meaningfully close that gap. Hispanic, Asian, and American Indian and Alaska Native women face similar screening inequities that a later start makes worse.

This is exactly why "average risk" guidance can fail real women. Your risk isn't average. It's yours.
Know your family history. Know your personal risk factors. Have the real conversation with your provider about when you should start screening, not just what a blanket guideline says.

You are not a statistic. You are not the average of a bell curve. Advocate for the screening that's right for your body.

Metabolism is a hormone conversation. Thyroid tells your cells how fast to burn energy. Cortisol decides whether you store fat or use it. Insulin controls whether sugar becomes energy or gets stuck in storage. Estrogen and testosterone influence muscle mass, which is directly tied to how much you burn at rest.

When one of these is off, the whole system compensates. That's why the same diet and workout that used to work suddenly stops working. That's why you can eat "clean" and still feel stuck.

You are not broken. You are not lacking discipline. Your hormones changed, and your approach has to change with them.

Root cause medicine means looking at the whole system instead of blaming the scale.

Save this if you've ever felt like your body stopped listening to you.

My guest last week, Dr. Jenn Simmons, told a story on the podcast that stopped me in my tracks.

She was about to do a biopsy when the patient looked at her and said, "Are you gonna do my biopsy or not?"

No conversation. No plan. Just get it done.

That was the day Dr. Jenn resigned.

Because she realized something a lot of us in medicine eventually learn the hard way. Not everyone wants to help themselves. Some people just want the pill. The procedure. Whatever makes the problem go away without them having to change a thing.

Here's what she said next, and it's stuck with me ever since.

You cannot make anyone healthy. Not a doctor. Not a diagnosis. Not a pill.

Health happens at home. In the stuff you do every single day. Physical. Mental. Emotional. Spiritual.

The only person who can make you healthy is you.

When stress stays high, your body converts thyroid hormone into reverse T3. Reverse T3 slows your metabolism, lowers your energy, and puts you into survival mode.

If you're stuck in burnout, your thyroid isn't failing you. It might be protecting you.

Save this if your energy has felt unpredictable lately.

I had to face something uncomfortable.

For the longest time, I thought being strong meant not needing anyone. Being independent. Easy. Low maintenance.

But what I was really doing was pushing my husband away every time he tried to care for me.

The moment I finally put words to it, I made myself stop talking and simply wait. Twenty-two seconds of silence that felt like forever.

Then he looked at me and said: "So... are you finally going to let me take care of you?"

That one question changed the way I saw strength.

Sometimes the strongest thing you can do isn't carrying everything on your own. It's letting someone who loves you show up for you.

If you've ever been the person who says, "I'm fine," even when you're not, this conversation will stay with you.

Fat cells aren't just sitting there minding their business. They're metabolically active: releasing inflammatory cytokines, shifting insulin signaling, and influencing your estrogen metabolism.

This isn't about shame. It's about understanding what's actually happening at the cellular level. Common does not mean healthy.

Save this for the next time someone tells you extra weight is "just cosmetic."

"I love you. And it would be dishonorable to commit when I can't give you what you need."

Most people hear that and think it's rejection...but what if it's actually respect?

We spend so much time trying to convince the people we love to want different things. To want marriage. To want children. To want the same future we do.

And when they don't, we often assume they just aren't trying hard enough.

But loving someone doesn't change what they truly want. And asking someone to promise a future they know they can't give isn't love, it's hope.

Sometimes the kindest thing someone can do is be honest before making a commitment they can't keep.

It's not the conversation most people want to have, but it might be the one that saves both people years of heartache.
