Not every transformation happens all at once.

This before and after represents a journey that took place over nearly two years.

When this patient first came to Dr. Rihani, she knew what bothered her most, but she also had to consider the amount of recovery time that worked with her life. The final result from our most recent case study.

This patient is now six months post-op from a lower blepharoplasty with fat transfer, performed by Dr. Rihani.

Rather than simply removing the fat beneath the eyes, Dr. Rihani also restored volume through the mid-cheek to create better support and a smoother transition from the lower eyelid into the cheek.

The result is a refreshed, more youthful appearance that still looks natural and balanced.

Not all under-eye bags should be treated the same.

Many people think the solution is simply removing the fat beneath the eyes. In reality, that can sometimes create a hollow or overdone appearance.

In this case, Dr. Rihani explains why the best approach is often a combination of a lower blepharoplasty and fat transfer.

As we age, it’s common to lose volume in the midface. Without that support, the fat beneath the eyes can become more noticeable, creating the appearance of tired, puffy under-eyes. By carefully repositioning and restoring volume with your own fat, the goal is to create a smoother transition from the lower eyelid to the cheek for a natural, refreshed appearance.

As part of our Patient Coordination Team, Shelly helps guide patients through every step of their surgical journey. From answering questions and coordinating surgery to making sure every detail is in place, she’s committed to creating a seamless, personalized experience from the very first conversation.

With more than 20 years of experience in facial plastic surgery and a Bachelor’s degree in Business Administration, Shelly brings both knowledge and compassion to every interaction. Since joining the Facial Plastic Surgery Institute team in 2017, she’s become known for her warm personality, quick wit, and genuine ability to make patients feel at ease.

Originally from East Texas, Shelly loves building lasting relationships with patients and helping them feel informed and supported throughout the process.
