When sciatica won’t go away, the problem may not be where you think it is.

Pain that starts in the buttock and travels down the back of the leg is often assumed to be coming from the lower back. But that isn’t always the case.

One frequently overlooked possibility is piriformis syndrome—a condition involving a small muscle deep in the buttock that can irritate the sciatic nerve and produce symptoms that closely resemble other causes of sciatica.

After 30 years in chiropractic practice, one of the things I’ve learned is that successful treatment starts with identifying what is actually causing the symptoms—not simply treating where it hurts.

Waking up with back pain? Your mattress may be part of the problem—but it may not be the whole story.

After more than 30 years of treating patients with back pain, one pattern I’ve seen repeatedly is that what happens during the 6–8 hours we spend sleeping can have a significant effect on how the spine feels and functions the next morning.

Sleep position, spinal alignment, mattress support—and even how you get out of bed—can all play a role.

When numbness, tingling, weakness, or pain won’t resolve, the problem may not be where you think it is.

In some cases, a nerve can be compromised at more than one location along its pathway—for example, at the neck and again farther down the arm. This is known as Double Crush Syndrome, and it can help explain why treating only the area where symptoms are felt may not produce lasting results.

Understanding where a nerve is being affected—and whether there is more than one point of irritation—is an important part of properly evaluating persistent nerve symptoms.

Poor ergonomics is probably the number one reason people end up having to see a chiropractor for neck pain.
