Forward Head & Altered Posture - Biomechanical
Breakdown
This photo illustrates a forward head posture with the resulting postural compensation. When the head translates forward, the neck experiences increased flexion called forward head posture.
To prevent the head from falling forward, neck extensor muscles become overactive and tight, while the deep neck flexors weaken. This imbalance increases compressive load on all the neck joints and discs.
As the upper back shifts rearward to maintain balance, the thoracic extension muscles and scapular stabilizers become exhausted. The chest muscles shorten, restricting rib cage expansion and altering breathing mechanics. Abdominal muscle imbalance develops, where upper internal obliques may dominate while external obliques become elongated and weak.
At the hips, a forward shift with posterior pelvic tilt flattens the lumbar spine. This reduces lumbar shock absorption and increases strain on the back. Hip flexors weaken, while hamstrings become short and tight, pulling the pelvis further out of normal alignment.
Down the chain, the knees often move into hyperextension as a compensatory strategy to maintain the center of gravity.
This increases stress on posterior knee structures and reduces efficient force transmission during standing and walking.
