CDS is a Medicaid based program which provides attendant services to individuals with disabilities, enabling them to live independently. Participants in the CDS program are trained to hire their own attendants to assist them with daily tasks.
What type of daily tasks might attendants be assisting in?
The tasks may include: Bathing; Toileting; assisting with dressing, Obtaining Medication; Meal Preparation; Meal Cleanup; Housekeeping; Laundry; and Essential Transportation.
What are the goals of CDS programs?
The CDS Program is designed to assist individuals with disabilities to receive the services they need to remain independently in their home instead of living in a more restrictive institutional environment.
How is Family First Service CDS Program designed?
Family First Service, LLC (CDS) program is consumer directed. One main component of consumer-directed in the CDS program is that the individual who receives CDS is the employer of the attendant. CDS participants are also responsible for locating, hiring and managing their own attendants.
A qualified friend or family member may be hired as an attendant, with the exception of a spouse/legal guardian or anyone under the age of 18.
To qualifies for CDS services, a person/Consumer must meet the following:
Medicaid eligible (MO HealthNet eligible)
18 years or age or older
Able to direct their own care
Capable of living independently with the services of CDS
Require at least a nursing facility level of care
Must have a physical/ mental disability
What geographical regions does Family First Service (CDS) serve?
Family First Service serves CDS Participants in the following regions: Jefferson County, St Louis City, St. Louis County, Lincoln County, St. Charles County, Warren County, Warrenton County and a few surrounding areas.
How is DSDS funded?
CDS is funded by MO HealthNet (Medicaid). DSDS participants must be Medicaid eligible in order for the CDS Attendant to be paid. The DSDS Participant's approved and verified Attendant services are billed to MO HealthNet according to the Care Plan authorized by DHSS.
Individuals on Medicaid will need to meet Missouri Division of Senior and Disability Services (FAMILY FIRST SERVICE) requirements. DSDS will complete an individual's needs and forward a personalized care plan to FAMILY FIRST SERVICE, LLC Once a care plan is received, Family First Service, LLC will contact you to set up the Consumer Training to get services started.
