✔️ Protect the resident's privacy and confidentiality by following HIPAA guidelines.

✔️ Monitor for signs of infection, skin irritation, leakage, or changes that should be reported immediately.

✔️ Ensure catheter bags remain below bladder level and tubing is free from kinks.

Accurate documentation is one of the most important responsibilities in healthcare. Every entry you make becomes part of the resident's permanent medical record and helps ensure safe, consistent, and high-quality care.

✅ DO:
• Chart promptly after providing care.
• Be accurate, factual, and objective.
• Document changes in a resident's condition immediately.
• Record only the care you personally provided.
• Use approved abbreviations and follow your facility's documentation policies.
• Sign or electronically authenticate every chart entry.

❌ DON'T:
• Chart care before it is completed.
• Guess or assume information.
• Copy another caregiver's documentation.
• Leave blank spaces or incomplete notes.
• Use white-out, erase, or alter documentation improperly.

• Speak respectfully and professionally.
• Protect every resident's dignity and privacy.

• Discuss resident information only with authorized healthcare team members in private.

❌ DON'T:
• Talk about other residents in front of anyone.

Dementia Doesn't Make Someone Difficult—It Makes Their World Different.

A resident living with dementia may yell, hit, kick, curse, or refuse care. While these behaviors can be challenging, they are often a form of communication—not intentional aggression.

Before reacting, ask yourself:

🧠 Are they in pain?

🧠 Are they scared or confused?

🧠 Are they overwhelmed by too much noise or activity?

🧠 Do they recognize who is helping them?

As caregivers, our responsibility is to respond with professionalism, patience, and respect.

✔️ Approach calmly from the front.

✔️ Introduce yourself every time.

✔️ Speak slowly and use a reassuring tone.

✔️ Give the resident personal space if they become upset.

✔️ Never argue, threaten, or try to "win" the conversation.

✔️ If it's safe to do so, step away for a few moments and try again later.

💜 Remember: You're not fighting the resident—you are fighting a disease that has changed how they experience the world.

Your calmness can become their comfort.

One Wrong Bite Can Cost a Life.

Choking and aspiration are not accidents to take lightly. They can lead to hospitalization, pneumonia, permanent injury, or death.

As a caregiver, there is no room for shortcuts.

⚠️ Sit the resident upright before every meal.

⚠️ Follow the prescribed diet exactly—never guess.

⚠️ Give small bites and allow time to swallow.

⚠️ If a resident coughs, chokes, or has a wet voice, STOP feeding immediately and follow your organization's emergency procedures.

Your job isn't just to feed a resident. Your job is to protect their life.

Every meal requires your full attention. Every resident deserves safe care. Every caregiver is accountable.
