Sterling Medical Center, a family-focused practice located in Sterling, KS is proud to provide quality healthcare to patients of all ages. Dr. James Decker, Dr. Kristina Darnauer, and Dr. Stacy Dashiell practice full-scope family medicine which includes pediatrics, obstetrics, urgent care and chronic disease management. They provide inpatient services, emergency care and perform deliveries at Rice

This program is a Medicare-supported service and is fully endorsed by our providers at Lyons Family Care and Sterling Family Care. It is designed to support qualifying patients by offering personalized, ongoing care coordination in partnership with your provider.

Some of the program benefits include:

*24/7 access to your dedicated care management nurse for questions/concerns and guidance when symptoms arise or conditions change
*Guidance on preventive care and screenings
*Medication review and adherence support
*Assistance with scheduling, managing appointments and medication refills

All interactions are shared with and guided by your provider and clinic care team to ensure seamless coordinated care.
