A hospice denial doesn’t always mean the patient didn’t qualify.

Sometimes, the decline was there. It just wasn’t clearly captured in the documentation.

So when a claim is denied because the record doesn’t support a six-month prognosis, the question becomes: Did the decline not happen, or did we fail to tell the story?
