Heel pain syndrome, also known as plantar fasciitis, is really small tears of a ligament-like structure that aids in stabilizing the arch and helping the foot push off during walking.
The first line treatments are custom molded foot orthotics, night splints, and rest. If the plantar fascia has significant tears, aggressive stretching may be interfering with healing.
When first line treatments are not helping, I recommend using ultrasound imaging in the office to evaluate for tears. The tears can be seen on ultrasound. If tears are present, regenerative medicine such as platelet rich plasma would be helpful to influence the body’s healing process.
If your plantar fasciitis has been present more than 3 months, it’s time for a detailed in office MSK Ultrasound to determine if tears are present.
