Crystal Rose Assisted Living Home Crystal Rose is a family-owned assisted living home located in Surprise, Az that has been providing compassionate care to elderly for over 18 years. b. c.

Our assisted living home is licensed for five residents which guarantees a more personalized care with emphasis on each person's individual needs. Services:
-24 hour care provided by certified staff;
-three well-balanced, nutritious meals a day;
-medication administration per physicians's written order;
-routine assessment of each resident by a registered nurse;
-assistance with bathing, dressing

and activities of daily living;
-laundry and housekeeping;
-Visits on premises available by podiatrist and other mobile doctors, physical and occupational therapists, hospice and hair-dresser;
-Cable TV and private phone lines available;
-Celebrations for special occasions;

-Transportation to local appointments available;
-Monthly fee is based on the level of care of the resident and type of room chosen;

2. Easier to adapt to and "navigate" especially for your loved ones with memory impairment since it's a home, not an institution.

If you know someone who needs 24 hour care and supervision for their loved one please let them know about our services. If the person moves in and is a resident of Crystal Rose for at least 2 months, we pay a referral fee of $2000.00 (in two monthly installments). We can also have this amount donated to a charity or church of your choice. When is placement necessary?

a. When the caregiver can no longer care for a loved one in his or her home. When no one is available to provide care and the individual cannot care for themselves. If one or more of the following conditions exist:

*Fear of being alone.

*Fear of falling.

*Difficulty with medication management.

*Cognitive impairment (short & long-tem memory).

*Poor judgment that leads to safety issues.

*Needs extra assistance with:
-- Feeding
-- Dressing
-- Bathing
-- Incontinence
