If you or a loved one isn’t quite ready to go home after a hospital stay, Transitional Care at Lynn County Hospital District may be the right next step.
Our program is designed for patients who need short-term skilled care to regain strength, independence, and confidence before returning home.
What makes our Transitional Care program different?

Our smaller facility allows for more personalized care than large hospitals.
• Fast response times
Our average call light response time is under 2 minutes.
• Comprehensive therapy services
We provide Physical Therapy (PT), Occupational Therapy (OT), Speech Therapy (ST), and Respiratory Therapy (RT) as part of your recovery plan.
• Hospitalist-led care
Patients are followed by our hospitalist team throughout their stay.
• Weekly care conferences
Personalized weekly meetings with the provider and the full interdisciplinary care team, with patient and family encouraged to participate (in person or by phone).
• Less stress for patients and families
Our smaller facility is easier to navigate, easier to locate your loved one, and offers parking within 75 feet of the front door.
Who may benefit from Transitional Care?
✔ Patients recovering after hospitalization
✔ Patients recovering from surgery
✔ Patients needing IV antibiotics
✔ Patients being discharged from larger hospitals who need additional support before safely returning home
Most patients stay about 2–3 weeks, and the majority return home stronger and more independent.
Admissions & Insurance
Patients may be admitted:
• Directly from LCHD inpatient care
• As a transfer from a larger hospital
• By referral from outside providers (with a qualifying 3-midnight hospital stay)
We accept Medicare, Medicaid, and most major insurances.
