Are you employed? Do you have a significant disability?

If you require assistance with at least two activities of daily living to attain and maintain employment, you may be eligible for a reimbursement of up to $1,800 per month to help offset the cost of your personal care assistance.

Participants must:
be employed
be at least 18 years of age
NOT be receiving SSI, SSDI or Medicaid Home & Community-based Services
be a U.S. Citizen or Legal Permanent Resident
Be a Florida resident
Require personal assistance services to perform at least 2 activities of daily living, such as ambulation, bathing, dressing, eating, grooming and toileting, as defined in Section 429.02, F.S.
