Community Hospice Ranks in TOP 5% IN THE NATION!!

Community Hospice Earns the 2025 SHP Best™ “Premier Performer” Caregiver Experience Award

Community Hospice has been recognized by Strategic Healthcare Programs (SHP) as a “Premier Performer” for achieving an overall caregiver and family experience score that ranked in the top 5% of all eligible SHP clients for the 2025 calendar year. The annual SHPBest™ award program was created to acknowledge hospice providers that consistently provide high quality service to families and caregivers of patients receiving hospice care. Award recipients were determined by reviewing and ranking the overall CAHPS Hospice caregiver experience score for more than 1,800 hospice providers.

At Community Hospice, quality, patient-centered care isn't just something we talk about—it is at the heart of everything we do.

We're proud to introduce Susan Lambdin, RN, CHPN, our Agency Director.

Susan has dedicated the majority of her nursing career to hospice care and has served in many roles throughout her journey, including Hospice Case Manager, Clinical Director, and Agency Director.

Her experience has given her a unique understanding of hospice from every perspective—from providing hands-on care to patients and families to leading the team that makes that care possible. She leads with a strict adherence to compliance and quality care while making sure every patient is treated with dignity, compassion, and respect.

Her years of hospice experience, leadership, and commitment to our community are an important part of what makes Community Hospice different.

Meet Our Community Hospice Nurse Practitioner

At Community Hospice, exceptional care is delivered by exceptional people—and we're proud to introduce DeRay Williams, CRNP. As our Hospice Nurse Practitioner, DeRay provides compassionate, expert care to patients in the comfort of their own homes. He works alongside our physicians, nurses, and interdisciplinary team to assess patients at the bedside, manage symptoms, address changing medical needs, and help ensure every patient receives the comfort, dignity, and individualized care they deserve.

In addition to his role with Community Hospice, DeRay has a thriving medical practice in Tallassee, where he is known throughout the community for his thorough attention to detail, clinical excellence, and genuine commitment to his patients. Whether caring for patients in his medical practice or making hospice home visits, DeRay believes in treating every person with compassion, respect, and dignity.

Meet Our Medical Director

At Community Hospice, compassionate care begins with compassionate leadership.
We're proud to introduce Dr. Pamela Pollard Tuck. A resident of Macon County, Alabama, Dr. Tuck brings years of experience and a heartfelt commitment to caring for patients and families in the communities in which she grew up.

In addition to serving as Medical Director, she practices family medicine at Baptist East Medical Center's Taylor Medical Complex in Montgomery.
Dr. Tuck works closely with our nurses and interdisciplinary team to ensure every patient receives individualized, compassionate care that honors their wishes and supports their loved ones.
At Community Hospice, we believe hospice is about more than medical care—it's about dignity, comfort, and walking alongside families during life's most meaningful moments.

We are proud to be a small, community-based hospice serving only three Alabama counties, Elmore, Macon and Tallapoosa.

Our nurses, aides, social workers, chaplain, volunteers, and support staff live right here in the communities we serve. When you call us, you are talking to neighbors who care deeply about the people and families around them!
