Vulvar pain has causes.

Some of the causes we know a lot about and these are considered “specific disorders” according to the Consensus Terminology on Vulvar Pain
(2015). Others, we are learning more about and the research is not super strong on them; we lump these into “vulvodynia” and recognize that there are “potential associated factors.”

Vulvodynia has causes, we just don’t have all the research on them yet to be so definitive, but we will get there! Someday, there will be no term known as
“vulvodynia” because we will know all the causes of v***ar pain.

Here are causes and “potential associated factors” for Vulvar Pain and Vulvodynia:

Hormonal- lack of estrogen and androgens related to menopause (called Genitourinary Syndrome of Menopause), breastfeeding (Genitourinary Syndrome of Lactation), antiandrogen and antiestrogen medications (combined oral contraceptives, spironolactone, tamoxifen).

Musculoskeletal- Hypertonic/overactive pelvic floor muscles, myofascial and biomechanical causes (common with hypermobility)

Inflammatory- lichen sclerosus, lichen pla**s, plasma cell vulvitis, desquamative inflammatory vaginitis (DIV), cytolytic vaginosis (CV), Mast Cell Activation (MCAS)

Infectious- recurrent v***ar candidiasis, tinea cruris, bacterial vaginosis (BV), aerobic vaginitis (AV), ureaplasma

Nerve-related- neuroproliferation (too many nerve endings in the tissue), spinal pathology (Tarlov cyst, annular tear, disc heriation), nerve compression, nerve irritation (pudendal neuralgia, postherpetic neuralgia).

latrogenic/Trauma- postoperative pain, chemotherapy, radiation effects, hormone therapy for breast cancer, perineal tears, episiotomy, female ge***al cutting.

What I wish every OB/GYN knew about v***ar pain

Vulvar pain is not rare—and it’s not simple.

Things I wish were more widely understood:
• Pain with s*x is never something to dismiss
• Skin conditions are often under-recognized
• Hormones matter (especially postpartum + menopause)
• Patients are often dismissed before they get to the right diagnosis

We can do better—and patients deserve better.

Common Causes of Vulvar Inflammation

Lichen sclerosus- a likely autoimmune conditions causing thickening and scarring of the skin of the v***a and around the a**s

Lichen pla**s- an autoimmune condition involving activated T-cells that affects both outside skin as well as mucous membranes (inside mouth, vestibule and va**na), often causing red ulcers and scarring.

Psoriasis- an autoimmune condition that affects skin and nails. When present on the v***a, it causes red raised and thickened plagues on hair-bearing (mons p***s, l***a majora). It can be associated with superimposed infection, contact dermatitis, or lichen simplex chronicus.

Plasma cell vulvitis- a condition of unclear cause (possibly related to autoimmunity, lack of hormone, infection) that involves bright red shiny patches on the vestibule and inner l***a minora.

Desquamative Inflammatory Vaginitis (DIV)- excessive yellow sticky va**nal discharge that is very irritating to the v***a and vestibule, causing skin to become red, inflamed, and itchy.

Contact Dermatitis- a skin reaction to exposure to a chemical irritant or allergen, such as a v***ar product, wipes, toilet paper, soaps, creams, or ingredients in detergents or fabric softeners.

Vulvar candidiasis (yeast)- a fungal infection in the skin on the outside v***a, leading to redness, itching, rawness, and small papercuts called fissures. Think more “baby diaper rash” rather than “va**nal yeast infection.”

Lichen Simplex Chronicus- a thickening and tearing of the skin of the v***a due to repetitive scratching or rubbing, usually set of by a skin reaction due to an irritant/exposure, leading to an itch-scratch-itch cycle.

“Everything looks normal” is not a diagnosis.

Patients hear this all the time.

But here’s the problem:
Many v***ar conditions are microscopic, hormonal, or nerve-related.

That means:
• Exams can look “normal”
• Tests can come back negative
• And patients still have real symptoms

If you’re in pain, your experience matters—even if it’s not obvious on exam.

Good care goes beyond “I don’t see anything.”

Lichen sclerosus is often missed for years.

Not because it’s rare—but because the symptoms are dismissed.

Watch for:
• Itching (especially at night)
• Burning or tearing with s*x
• Skin that looks white, thin, or fragile
• Recurrent “yeast infections” that aren’t yeast
• Changes in anatomy over time

Early diagnosis matters—it can prevent scarring and long-term complications.

If something feels off, it’s worth getting evaluated by someone who knows what to look for.
