Happy Halloween: tonight's the night.

Garage open, one jack-o-lantern and a candy bowl loaded like it's the last shipment of the year, standing guard by the front step.

The forecast gave Tampa one clean, warm trick-or-treat night before the rain takes Sunday. Wishing everyone a safe, happy Halloween: check the candy, hold the flashlight, and enjoy every minute of it.

Three insurers sued CMS this week over their Medicare Advantage star ratings. Aetna claims $2 billion. Centene claims $448 million. Both say CMS kept applying quality measures a court already struck down for a competitor.

Here's the part nobody's saying out loud: the bonus pool isn't infinite. If Aetna wins its $2 billion, that money comes from somewhere, and quality bonus money moves through plans, hospitals and networks before it reaches a practice's contract rate.

So while the lawsuits argue about scorecards, the quietly interesting question is downstream: does your payer contract's 2028 rate conversation happen with a bonus pool that just got redistributed by a judge?

Most practices will learn the answer from the remittance, not the ruling.

Who on your team knows which of your payers has star-rating money in their contract rate right now? General information, not billing or legal advice.

The wind did some landscaping while we slept.

Sunday morning: one dead palm frond stretched across the driveway like it's waiting for a signature, three more parked along the grass edge, and a fine layer of palm dust on a car that was clean yesterday.

Nobody on the block will touch theirs until at least Tuesday. Not laziness. We're waiting to see if the wind is finished deciding.

Gusts to 35 on a bright 82-degree day is the trade we make for not having a winter. Worth it, mostly.

Last day of lacrosse season today.

Shin guards, cleats, sunflower seeds, the whole drill, and that cooler Florida sun that makes October the reward for everything August put us through.

If you're on a sideline somewhere today: enjoy it. The season's short, the sun's kind, and the kids are faster than we are. Hope everyone's having fun out there.

The first real cool front of the season arrives this weekend. By Sunday morning it's low 60s, which in Florida counts as a national holiday.

Expect every neighborhood to produce at least one person in a hoodie and boots, standing in the driveway not doing anything, just confirming it's real.

We get about two weeks of this a year. Go stand in it.

Stone crab season opens Thursday. Claws under 2⅞ inches go back to grow a replacement. Our Yorkie knows only the counter, mustard sauce and five days until the first crack.

Meet Kathy. She verifies coverage at the desk, gets the group number, repeats the group number, and discovers the group number is not actually the plan.

We made this video because it’s funny. We also made it because the useful part is painfully true: verify coverage before the visit, not at the desk.

Before the patient arrives:
• Confirm the plan is active for the date of service.
• Match the member ID to the correct payer and product.
• Check prior authorization before the patient is in the room.
• Keep the reference number, call date, and representative’s name with the chart.

Don’t be a Kathy. Watch the video, then see our tips post for the full checklist.

October 1 did not create one Medicaid change. It opened a state-by-state divide, and exposed how much implementation depends on administrative capacity.

Beginning October 1, federal matching funds for full-scope Medicaid and CHIP end for affected noncitizen categories. Each state must decide whether to continue that coverage with state-only dollars or end it. Federal reimbursement for emergency care for uninsured immigrants also falls. Where state-only coverage is not funded, hospitals and practices may absorb more uncompensated emergency care.

Montana begins enforcing its Medicaid work and community-engagement requirement October 1, months ahead of the federal January 1, 2027 deadline. Nebraska and Arkansas are already moving through earlier rollout and verification steps.

What do the early-moving states have in common? Not necessarily the toughest policy. Their systems were further along in income, status, and documentation verification. States that waited are now building that machinery.

For a practice, this changes the front desk. A patient may need different documentation at renewal. Eligibility may need to be checked before each visit, not assumed from the last appointment. If coverage changes during treatment, the practice needs a clear process for authorization, payer communication, patient notices, and unpaid balances.

One example: a well-child or prenatal visit scheduled under full-scope Medicaid may require a fresh eligibility check and confirmation of whether a state-only or other coverage pathway remains available before the next appointment.

The practical work this month is straightforward: identify patients in affected eligibility groups, confirm your state’s implementation date, and learn whether your state is funding state-only coverage. The policy is decided at the state level. The patient experience is shaped at the practice level.

General information, not billing, coding, legal, or reimbursement advice. Confirm requirements with your payer and a qualified professional.

Meet Kathy. She's the star of our new video, and she's having a rough one.

Kathy verifies coverage when the patient is already at the desk. She gets the group number, repeats the group number, and discovers the group number isn't actually the plan.

We made the video because it's funny. We're posting this because it isn't.

Don't be a Kathy. Before the visit:

1. Confirm the plan is active for the date of service, not just the day you call. For a January appointment, start asking in October.
2. Match the member ID to the correct payer and product. A group number is not a plan; one group can carry several products with different rules.
3. Check prior authorization before the patient is in the room. If a payer drops it, check again: dropping prior authorization does not drop medical-necessity review.
4. Save the reference number, call date, and representative's name with the chart.

Verify before the visit, not at the desk. The patient should never have to wait on hold or hear the mess.

Someone on the block put the inflatable skeleton up on October 1.

It's 91 degrees. By noon it's face-down in the grass, and the Yorkie has inspected the whole scene and filed no report.

Halloween in Florida is a negotiation. We decorate. The heat un-decorates. Repeat.
