Pelvic pain, pain with s*x, irregular bleeding, and fertility challenges can point to either endometriosis or pelvic inflammatory disease, and it is common for both to be considered when these symptoms show up.

What matters is that these two conditions have very different causes and require very different treatments. Understanding the distinction is an important step toward getting the right care.

"Does a pessary hurt?" is one of the most common questions we get.

The short answer: a properly fitted pessary should not hurt. Most people describe it as comfortable, supportive, and easy to forget about during daily activities. If something feels off, that is a sign the fit needs adjusting, not that pessaries are not right for you.

If you have endometriosis and are still in pain after trying birth control, medications, or surgery, there is a reason, and it has a name.

Endometriosis changes the muscles, nerves, and movement patterns of the body over time. Even when lesions are treated, those patterns often stay. Pelvic floor physical therapy works to address that layer of pain directly, and research shows it can make a real difference for pain, intimacy, and bowel and bladder symptoms.

After birth, estrogen levels drop, especially if you’re breastfeeding. This can contribute to pain with s*x, dryness, tissue sensitivity, and slow healing.

Vaginal estrogen works locally to support tissue health and comfort and is often used safely in the postpartum period when appropriate.

Pain with in*******se is common, but it’s not something you just have to push through.

If you use a pessary or have been told one might help, you might be wondering how long you can actually keep using it.

The answer is longer than most people expect. With the right fit and regular check-ins, a pessary can be used safely for years, even through major transitions like postpartum recovery and menopause. The device itself may need replacing every year or two, but that does not mean starting over.
